Not every doctor does. If you are employed on a straightforward salary with no additional income and no expenses to claim, your PAYE position may be perfectly manageable on its own. However, an accountant becomes genuinely useful once your situation includes locum income, private practice earnings, consultancy or medico-legal work, a partnership profit share, or significant professional expenses such as medical indemnity and subscriptions. In those cases, an accurate return depends on correctly identifying what is taxable, what can be claimed and how different income sources are reported together.
A doctor's return typically needs to bring together several elements: employment income and tax already deducted through PAYE, profits from self-employment such as locum or private work, and any other taxable income. On the expense side, items such as medical indemnity, professional body subscriptions, required training, specialist equipment and business travel may be relevant, but only where the rules and your circumstances support the claim. Because treatment depends on your employment status, income sources and structure, the return is prepared around your specific position rather than a standard set of assumptions.
NHS salary is usually taxed through PAYE, while private work, locum sessions and consultancy income are often reported through Self Assessment or, in some cases, through a limited company. The key is to reconcile the two accurately — matching payslips and P60 information with invoices and business records — so that nothing is reported twice and nothing is missed. Where private income is significant, we also look at whether your current structure and payment arrangements remain sensible.
Yes. Locum doctors often work through agencies or directly with trusts and practices, which means irregular income, multiple payers and expenses that vary month to month. In many cases, medical indemnity cover and professional body subscriptions can be claimed as allowable expenses where they are incurred wholly and exclusively for the purposes of your professional work. The exact position depends on how you work and how the cost relates to your income sources, which is why it is worth reviewing these costs rather than assuming. GP partners generally need partnership accounts prepared, profits allocated correctly between partners, and individual tax returns that reflect each partner's share plus any other income. Practice overheads, drawings and the timing of profit distributions all affect the personal tax position.
