It depends on your circumstances. If you are employed on a single NHS contract with no other income, your tax is largely handled through PAYE and an accountant may add limited value. However, if you have locum income, private practice or consultancy fees, medicolegal work, a partnership share, a limited company, or significant professional expenses to claim, an accountant can help ensure your returns are accurate, your expenses are treated correctly and your tax position reflects your actual working pattern. Many doctors find that the complexity grows gradually and their tax affairs need reviewing well before they realise it.
Locum work is usually treated as self-employment, which means you report your income and allowable expenses on a Self Assessment tax return rather than relying on PAYE alone. You will need to keep records of the shifts you work, the income received, and the costs you incur in carrying out that work, such as travel between sites, indemnity cover and professional subscriptions. If you also hold an NHS post, the two income streams are reported separately but taxed together, which is why it is important to reconcile the figures rather than treat them in isolation.
Common examples include medical indemnity premiums, professional body and registration subscriptions, mandatory training and continuing professional development that is required for your role, specialist equipment and professional books or journals, and business travel or mileage between work locations. The test is whether a cost is incurred wholly and exclusively for the purposes of your work, and the treatment of an expense can vary depending on whether it relates to employment, self-employment or both. Expenses that serve a dual purpose, such as ordinary commuting, are generally not allowable, so it is worth reviewing your spending rather than assuming everything can be claimed.
Your NHS salary is normally taxed through PAYE, while private practice, locum or consultancy income is usually reported through Self Assessment, but the two are added together to determine your overall tax liability. That means private income can push you into a higher rate band or reduce allowances, even though the tax on it was not deducted at source. Expenses that relate purely to private work are generally set against private income, and where costs relate to both, an apportionment may be needed. Getting this balance right is one of the more common areas we review.
VAT may become relevant once private income reaches a level where registration is required, and the position depends on the nature of the services you provide, since some medical services are treated differently from others for VAT purposes. Private practice also involves additional record keeping and invoicing requirements that need to be built into your systems. The sensible approach is to monitor your income and consider the VAT position before you cross a registration threshold, rather than dealing with it retrospectively.
A limited company can suit some doctors, particularly where there is substantial private, locum or consultancy income, but it is not automatically the better option. It brings additional compliance obligations, including statutory accounts, Corporation Tax returns and confirmation statements, along with rules around how you pay yourself and how expenses and benefits are treated. Whether it is worthwhile depends on your level of private income, your other employment, your pension position and your longer-term plans.
