We worked with a few different accountants before finally settling with Accotax, and we’re really glad we did. Their team is great. Abdul is responsive and easy to reach whenever I have a question, which makes a big difference. They take the time to understand how our business works and what our needs are. They are very knowledgeable, and when they don’t have the answer straight away, they take the time to look into it and get back to us quickly. Anum, who looks after our account on a day-to-day basis, including reconciling invoices and preparing the annual accounts, is amazing. She is professional, efficient, and follows the processes we have put in place together. She's also very quickin getting back to us whenever needed. We’re very happy to work with her and hope to continue doing so for as long as possible. Overall, Accotax has made our accounting experience much smoother and easier. We see them as a long-term partner as our business continues to grow. I would 100% recommend them.Posted on Google![]()

Accounting and Tax Challenges Doctors Commonly Face
Doctors often carry a combination of employment, self-employment and business income, each with its own reporting and record-keeping requirements. Some of the more common accounting and tax considerations include the following.
- Multiple income sources running side by side. NHS salary, private fees, locum shifts and consultancy work may all arrive through different routes and on different timescales, which makes it easy to lose sight of your total taxable income and harder to set aside the right amount for tax.
- Self Assessment alongside PAYE. Salaried doctors with additional income still need to report it, and payments on account can catch people out if the extra income is not planned for during the year.
- Working out allowable professional expenses. Medical indemnity, professional subscriptions, training and professional development, specialist equipment, and travel or mileage for work-related journeys can all be relevant — but only where the expense genuinely relates to your work and the rules are met. Claiming too much creates risk; claiming too little means paying more tax than necessary.
- Locum income and irregular working patterns. Shift-based work means income varies month to month, and expenses such as travel between sites, agency fees and equipment need to be recorded consistently if they are going to be claimed correctly.
- Choosing and maintaining the right business structure. Whether private work should sit in a sole trader arrangement, a partnership or a limited company depends on your level of income, how you work and your longer-term plans, and the answer can change over time.
- Partnership and practice finances. GP partners and doctors in partnership arrangements need accurate profit shares, partnership accounts and clear records, with personal tax consequences that flow directly from the partnership's results.
- Pension considerations. Annual allowance and contribution questions can affect higher-earning doctors significantly, and pension decisions often interact with overall tax planning in ways that benefit from joined-up advice.
- Keeping records organised and meeting deadlines. Receipts, invoices and bank records need to be maintained as you go. Leaving everything to the last minute increases the risk of missed income, unclaimed expenses and unnecessary pressure at filing time.

FAQs
It depends on your circumstances. If your only income is a straightforward NHS salary taxed through PAYE and you have no additional income or claims, your tax position may be relatively simple. However, many doctors do have additional income — private fees, locum shifts, consultancy, medico-legal work, teaching or rental income — and that usually means a Self Assessment return, careful record keeping and decisions about allowable expenses. Doctors in partnership, running a practice or operating through a limited company almost always benefit from specialist support. A short conversation is usually enough to establish whether you need ongoing help or just one-off advice.
Your NHS salary is normally taxed through PAYE, but it still needs to be included on your Self Assessment return alongside any private medical income, locum earnings or consultancy fees. Depending on your total income and the tax already deducted, you may have additional tax to pay, and payments on account can apply. The practical challenges are making sure all income is captured, that allowable expenses relating to private and additional work are claimed correctly, and that you have set aside enough to meet the liability when it falls due.
Common examples include medical indemnity or professional indemnity insurance, professional body subscriptions, costs of training and professional development that relate to your existing work, specialist equipment and uniforms where appropriate, and travel or mileage for genuinely work-related journeys. The rules are specific, and treatment can differ depending on whether the expense relates to employment, self-employment or a limited company. Some expenses that seem obviously claimable are not, and others that doctors often overlook are. We look at your actual expenditure rather than working from a general list.
Locum doctors often work as self-employed sole traders, which means income and expenses are reported through Self Assessment rather than taxed under PAYE. That brings obligations around bookkeeping, record keeping and, in some cases, registering for VAT. Because locum income is irregular, planning for tax during the year matters more than it does for a fixed salary. Some locums choose to operate through a limited company, and whether that is worthwhile depends on your level of income and how you work — it is a decision worth reviewing rather than assuming.
Not automatically. A limited company can be appropriate for doctors with substantial private, locum or consultancy income, and it changes how you pay tax, how accounts and Corporation Tax returns are prepared, and how you take money out of the business. But it also brings additional compliance and administrative responsibilities. For others, sole trader or partnership arrangements remain more suitable. The right answer depends on your income levels, your mix of NHS and private work, your pension position and your longer-term plans, so it is worth examining properly before making a change.
Yes. Doctors in partnership — including GP partners and those in shared private practices — need partnership accounts, agreed profit shares and a clear understanding of how partnership profits feed into each partner's personal tax position. We prepare the accounts and returns, keep the underlying bookkeeping organised, and explain the personal consequences of the partnership's results so there are no surprises at filing time.
We work with clients remotely as well as through arranged meetings, using secure digital records and communication, which suits doctors whose working patterns are unpredictable. Changing accountants partway through the year is usually straightforward — we handle the handover with your existing adviser, obtain the necessary records and authorisations, and make sure nothing is missed in the transition. It is worth starting the process before a filing deadline rather than closer to it.
It begins with a conversation about how you work and what you need. We will ask about your income sources — NHS, private, locum, consultancy or a combination — your current accounting arrangements, your business structure and any concerns you have about tax or record keeping. From there we review your position and explain what support would be appropriate, what it involves, and what we need from you. You are under no obligation to proceed until you are comfortable with the approach.
