A doctor with a single NHS salary and no other income may have relatively straightforward affairs, although pension and expense questions can still arise. Once you add private fees, locum income, consultancy work, a practice, or a partnership, the reporting becomes more involved and the scope for missed expenses or incorrect treatment grows. An accountant who regularly deals with medical income can help you report each source correctly and keep the administrative side manageable.
If you have untaxed income, such as private fees, locum earnings or consultancy payments, you will usually need to file a Self Assessment return even though your NHS salary is taxed through PAYE. Both need to be brought together on the return, with the private income and any allowable expenses reported alongside your employment details. Keeping accurate records of private invoices, agency statements and business costs throughout the year makes the return far quicker to prepare and reduces the risk of errors.
Expenses must be incurred wholly and exclusively for your work, and what qualifies depends on your circumstances. Locum work is usually treated as self-employment, which means registering with HMRC, keeping records of income and expenses, and filing a Self Assessment return. Because you are responsible for your own tax and National Insurance rather than having them deducted, budgeting for the eventual bill matters, and allowable costs such as travel between contracts, indemnity cover and equipment can be claimed where they meet the rules.
