It depends on your circumstances. If you are salaried with tax handled entirely through PAYE and no other income, your tax affairs may be relatively simple. However, once you have private fees, locum income, consultancy or medico-legal work, investment or rental income, or a tax position affected by pension contributions, Self Assessment becomes part of your year. An accountant can help you report the right figures, claim the expenses you are entitled to, and avoid unwelcome correspondence from HMRC. Many doctors also value the reassurance of someone checking their position each year rather than reviewing it only when something goes wrong.
Your NHS salary is normally taxed through PAYE, so it is largely dealt with before it reaches you. Private fees, locum payments and consultancy income usually are not, which means they need to be reported through Self Assessment. In practice, you bring all your income sources together on the return, claim allowable expenses against the self-employed side, and the tax already paid through PAYE is credited against your overall liability. Whether a particular cost is allowable depends on the rules and on whether it relates to your employment or self-employment, and some employment expenses must be claimed through HMRC rather than your tax return.
