Not every doctor does. If all of your income is taxed through PAYE and you have no private work, no self-employment and no additional income, your tax position may be straightforward. An accountant tends to become valuable as soon as something sits outside that picture — private fees, locum shifts, consultancy or medico-legal income, a partnership profit share, rental income, or a limited company. At that point, questions about what is taxable, which expenses are allowable and how payments on account work become harder to answer confidently without specialist support.
It depends on your wider circumstances. NHS employment income is normally taxed through PAYE, but if you also receive private medical fees, locum payments, consultancy income, partnership profits or certain untaxed income, you may need to register for Self Assessment and declare everything together. There are also situations where HMRC asks a taxpayer to file a return for other reasons. The right answer depends on your individual position, so it is worth checking before a deadline approaches rather than afterwards.
NHS employment income is generally taxed through PAYE, with tax deducted at source and National Insurance contributions handled by the employer. Private medical income is often self-employment or miscellaneous income that you report yourself, with tax paid through Self Assessment, potentially alongside payments on account. Because the two are taxed through different mechanisms, the interaction between them — and the effect on your overall tax position — is one of the areas where doctors most often benefit from advice.
Commonly considered items include medical indemnity insurance, professional body subscriptions, registration and revalidation costs, job-related training and continuing professional development, specialist equipment, professional books and journals, and travel between work sites where it is genuinely business travel. Whether each item is allowable depends on your employment status and the purpose of the cost — expenses incurred wholly and exclusively for a trade or professional practice are treated differently from those reimbursed or claimable through employment. Good records and a clear explanation of each cost make the position much easier to support.
Locum work is often treated as self-employment, which means reporting income and expenses through Self Assessment, keeping records of invoices and payments, tracking allowable costs such as indemnity cover, transport between sites, and administrative support, and budgeting for tax that is not deducted at source. Where locum work sits alongside NHS employment, the combination can affect how much tax you pay overall. Some locums operate through a limited company, and whether that is beneficial depends on your contracts, income level and personal circumstances.
There is no universally better option. Sole trader or partnership status is simpler administratively and suits many doctors with modest private or locum income, while a limited company can offer different tax treatment and flexibility over how profits are extracted, but brings additional obligations such as statutory accounts, Corporation Tax returns and, in some cases, VAT registration and payroll for directors or staff. The right structure depends on your income sources, how much you earn, how you want to take money out, and your longer-term plans.
