That typically includes preparing and filing Self Assessment returns where you have private income, locum earnings or medico-legal fees, preparing annual accounts and Corporation Tax returns for limited companies and incorporated practices, partnership returns for practices run by more than one practitioner, bookkeeping, payroll for practice staff, VAT advice where services fall outside the medical exemption, and tax planning around business structure and pension contributions. The practical aim is accurate filing, correct tax treatment and financial information you can act on.
It suits anyone earning from healthcare work. Not everyone does. If your only income is taxed through PAYE and there is nothing else to report, you may not need a return. Most medical and dental care is exempt from VAT, which means it does not count towards the registration threshold. However, not everything a practice does is exempt. Cosmetic treatments that are not aimed at protecting, restoring or maintaining health, together with retail sales such as products, supplements or non-medical services, are generally standard-rated. If your taxable turnover from those activities exceeds the registration threshold, you may need to register, and mixed exempt and taxable activity can bring partial exemption rules into play. What matters more than location is that records are passed over promptly and questions are raised early, so nothing is left until the filing deadline.
Keep invoices and receipts for all income and expenditure, bank and credit card statements, mileage logs, payslips, superannuation statements, pension contribution records, professional registration and indemnity renewals, and any contracts or associate agreements. HMRC generally expects records to be kept for at least six years from the end of the relevant accounting period, and longer in some situations. Digital record keeping is usually easiest to maintain and simplest to hand over, and it reduces the risk of missing claimable expenses or having to estimate figures at year end.
