We worked with a few different accountants before finally settling with Accotax, and we’re really glad we did. Their team is great. Abdul is responsive and easy to reach whenever I have a question, which makes a big difference. They take the time to understand how our business works and what our needs are. They are very knowledgeable, and when they don’t have the answer straight away, they take the time to look into it and get back to us quickly. Anum, who looks after our account on a day-to-day basis, including reconciling invoices and preparing the annual accounts, is amazing. She is professional, efficient, and follows the processes we have put in place together. She's also very quickin getting back to us whenever needed. We’re very happy to work with her and hope to continue doing so for as long as possible. Overall, Accotax has made our accounting experience much smoother and easier. We see them as a long-term partner as our business continues to grow. I would 100% recommend them.Posted on Google![]()

The core disciplines are the same — accounts, tax returns, payroll, VAT and bookkeeping — but the detail differs. Healthcare professionals frequently have a mix of NHS salary or pension income, locum or private fees, and possibly practice profit, all of which need to be reported together on a Self Assessment return. VAT treatment depends on whether a service is exempt or taxable, which is rarely a simple yes or no across an entire practice. Expenses range from indemnity insurance and professional registration to equipment and home-working costs, each with its own apportionment rules. Working with an accountant who deals with these patterns regularly means fewer questions get missed and less time is spent explaining how your income is earned.
Not always, and it is worth being honest about that. If your only income is taxed through PAYE, your tax code is correct and you have no untaxed income, savings interest above the allowance, or complicated expenses, an accountant may not add much. The position changes if you start locum work, receive private fees, hold a directorship, rent out a property, or receive pension income that triggers an annual allowance charge. At that point a return is usually required and payments on account come into play, which is where specialist help tends to pay for itself.
Locum and private income is generally reported on a Self Assessment tax return, with tax and Class 4 National Insurance calculated on the profits after allowable expenses. If you also have NHS salary, the tax already deducted through PAYE is credited against the total bill rather than being the final word. Because the return is filed and paid by 31 January following the end of the tax year, and payments on account may be due in January and July, the cash flow impact can be significant. Keeping records throughout the year and reviewing the estimated liability in advance makes that far easier to manage.
Yes. This is a common situation for practices offering both standard clinical work and services such as cosmetic treatments, medico-legal reports, occupational health assessments, retail sales or room hire to other practitioners. You may be able to treat some services as exempt while others are taxable, which affects registration, invoicing and how much input VAT can be recovered. It usually requires records split by service line and, in some cases, a partial exemption calculation. Reviewing this before turnover approaches the registration threshold gives you more options than dealing with it afterwards.
Partners are taxed on their share of partnership profit rather than on the money they physically withdraw, so drawings and tax are separate matters. Each partner reports their share on their own Self Assessment return, and the partnership submits its own return. Where partners have different profit shares, work patterns or pension arrangements, planning the split and the timing of drawings matters. Practice owners trading through a limited company face a different set of decisions around salary, dividends and Corporation Tax. We look at both structures and explain the trade-offs rather than assuming one is always better.
Keep records of all income, including NHS statements, locum invoices, private fees and insurer payments, alongside receipts and invoices for costs such as indemnity cover, professional body fees, registration, CPD and study, clinical equipment, software, accountancy fees, and business travel. If you work from home, keep evidence of the space used and the associated bills. If you employ staff, retain payroll records and pension documentation. Digital records are generally easier to maintain, and a simple monthly routine of filing bank transactions and receipts avoids a rush before the filing deadline.
Not necessarily. Many healthcare professionals operate effectively as sole traders or through a partnership, and incorporation is not automatically tax-efficient — it depends on your profit level, how you take money out, your pension arrangements and whether you need limited liability. A limited company brings additional filing and record-keeping obligations, including annual accounts, a confirmation statement and Corporation Tax returns. We can compare the likely outcomes under each structure based on your own figures before you commit, rather than applying a general rule of thumb.
Yes. Most accounting work is handled entirely through digital records, secure document sharing, email and video or telephone calls, which suits clinicians with irregular hours and limited spare time during the working day. You can send information when it suits you, and we can arrange calls around clinic lists or surgery times. If you would prefer to meet, that can be arranged as part of getting started. The important thing is that you have a clear point of contact and know how to reach them.
