Not everyone does. If all your income is taxed through PAYE and there is nothing else to report, a return may not be required. However, healthcare professionals often have something that triggers one: self-employed income, locum fees, private practice earnings, partnership profits, untaxed savings or rental income, or income over £100,000. In those cases a return is usually needed even where tax has already been deducted elsewhere, and it is worth confirming your position rather than assuming.

NHS salary or pension income is usually taxed through PAYE, while private fees are often paid gross, so the two need to be brought together correctly on a tax return. Payments on account are a frequent issue here, because private income can push someone into a larger bill than expected. We also look at whether the private work is best carried out as a sole trader or through a company.
Commonly claimed items include professional indemnity insurance, registration and professional body fees, CPD and training that maintains existing skills, clinical equipment, specialist clothing and uniforms, and business travel between sites. Home office costs, phone and internet use, and vehicle expenses usually require a reasonable business proportion to be worked out. The rules are specific, so we review claims before they go into a return to make sure they are properly supported.
Some healthcare services are exempt from VAT, but not every service a clinic provides falls into that category, and mixed NHS and private work can complicate matters. Once turnover approaches the registration threshold, the question becomes how much of your income is exempt, how much is standard-rated, and whether partial exemption rules apply. It is better to review this in advance than to discover a registration obligation partway through a year.

Much of the day-to-day accounting relationship now runs digitally, with records shared through cloud bookkeeping software, documents uploaded securely, and meetings held by phone or video where that is more convenient than travelling. What matters is not the postcode of your accountant but whether they understand your income, expenses and obligations.
