In practice that usually means Self Assessment for self-employed clinicians and locums, annual accounts and Corporation Tax for incorporated practices and clinics, bookkeeping, VAT registration and returns, payroll for clinical and administrative staff, pension contributions, expense reviews and business structure advice. For healthcare clients, the added value is understanding how income arrives — NHS contracts, private fees, insurance work, associate shares — and making sure each stream is recorded and taxed correctly.
They are suitable for anyone earning from healthcare work, whether as an individual or a business. That includes GPs, dentists and dental associates, pharmacists, consultants, physiotherapists, practice nurses, carers and support workers who are self-employed, as well as medical practices, dental surgeries, private clinics, care providers and healthcare companies with employees. If your income or your business involves clinical services, regulated healthcare activity or a mix of NHS and private work, the accounting questions tend to be similar.
Not everyone does, but many healthcare professionals do. If you are self-employed, a partner in a practice, a company director, or you receive income that is not fully taxed at source — private fees, locum payments, rental income or significant savings and dividend income — a return is normally required. If your only income is taxed through PAYE and nothing else applies, you may not need one. It is worth checking your position rather than assuming, because penalties for not filing can apply even where no tax is due.
They are reported together on your tax return, but they behave differently in practice. NHS income usually follows contracts, sessional payments or pensionable employment with tax deducted through payroll, while private and locum income is often paid gross and needs to be recorded carefully with the associated expenses. Where both exist, payments on account, pension annual allowance considerations and the timing of receipts can all affect what you owe, so keeping the two clearly separated in your records makes the return much more accurate.
Common claims include professional indemnity insurance, registration and regulatory fees, medical equipment and its maintenance, continuing professional development, specialist clothing such as scrubs where required for work, professional subscriptions, and travel between work sites. Where you use part of your home for administration or a consultation room, a reasonable proportion of associated costs may be claimable. The key is that expenses must be incurred wholly and exclusively for your work, be properly documented and stand up to scrutiny if HMRC asks about them.
It depends on turnover and what you supply. Many healthcare services are exempt from VAT, but a business can become liable to register once its taxable turnover crosses the registration threshold, and mixed businesses — for example a pharmacy with retail sales, or a clinic selling non-clinical products — may need to account for VAT on part of their activity. Partial exemption rules can then apply to the VAT you reclaim on costs. It is worth reviewing your position before you approach the threshold rather than after.
Yes. Payroll support typically covers salaried receptionists, nurses, assistants and other employees, including payslips, PAYE, National Insurance and pension contributions. Locums, associates and sessional clinicians are a different matter, because their employment status determines whether they should be paid through payroll or treated as self-employed suppliers. Getting that classification right matters for both the practice and the individual, so we review the working arrangements before setting the payroll up.
