The work covers the same core areas as any accountancy service, applied to the way healthcare income is earned. In practice that means Self Assessment returns combining NHS pay with private fees, locum work or medico-legal income, annual accounts and Corporation Tax returns for incorporated practices and clinics, bookkeeping, payroll for practice staff, VAT registration and returns, and tax planning around business structure, drawings and dividends. It also includes the interpretive side: explaining what is and is not allowable, flagging deadlines, and dealing with HMRC correspondence on your behalf.
Not always, but more healthcare professionals need to file a return than expect to. If you have private patient fees, locum income, medico-legal work, rental income, dividends, significant savings interest or you need to claim relief on professional expenses and pension contributions, a Self Assessment return is likely to be required. A short review of your circumstances is usually enough to confirm whether you need to file and what could reasonably be claimed.
NHS salary is taxed through PAYE, with tax deducted at source. Private fees, locum work and self-employed clinical income are reported through Self Assessment, and tax is usually paid via payments on account in January and July rather than monthly. Expenses that are incurred wholly and exclusively for that work can be deducted, which is why accurate records and a sensible bookkeeping routine matter. Where the two income types overlap in the same year, they need to be reported together so the correct rates of tax apply.
Common examples include professional registration and regulatory fees, professional indemnity and medical defence subscriptions, CPD and required training, specialist equipment and instruments, journal subscriptions, business mileage between sites, and a proportionate share of home costs where you carry out administrative work. Ordinary clothing and routine commuting are not allowable. Because the rules depend on your specific role and contracts, it is worth reviewing your actual outgoings rather than relying on a generic list.
Medical care provided by registered health professionals is generally exempt from VAT, but not everything a clinic does falls into that category. Cosmetic treatments, retail sales, some non-medical reports and certain wellness services can be standard-rated. Where you provide a mixture, your taxable turnover must be measured against the registration threshold and partial exemption rules may apply to the VAT you can reclaim. Getting this wrong can be costly, so the mix of supplies should be reviewed carefully before registration decisions are made.

Partners are taxed as self-employed individuals on their share of the partnership's profits, and the partnership itself submits a separate return. That means your own Self Assessment depends on both your profit share and any other income, and Class 4 National Insurance applies to the self-employed element. Practical points such as drawings versus profits, capital allowances on equipment and whether incorporation would be beneficial are all worth reviewing periodically rather than only at the start.
