The difference is in the detail of how healthcare income works. A specialist accountant understands that NHS pensionable pay, private fees, locum work and practice profits are reported differently, that medical services usually sit outside the scope of VAT while cosmetic and retail elements may not, and that partnership profit sharing in a GP or dental practice needs careful handling. The compliance work — accounts, tax returns, payroll and VAT — is similar in name, but the judgement applied to it is specific to healthcare.
It can. Both streams are taxable, but private fees are usually self-employed income reported on Self Assessment, while salaried NHS work is taxed through PAYE. If you also receive NHS pension income, that adds another layer. What matters is recording each source accurately and making sure expenses are allocated sensibly between them, so the return reflects your real position rather than an estimate.
Many healthcare professionals do, even when they also pay tax through PAYE. If you receive private fees, locum income, untaxed savings or rental income, are a partner in a practice, or have income above the reporting thresholds, a return is generally required. You need to register with HMRC by 5 October following the end of the tax year in which the income arose, and the online return and payment are due by 31 January.
Costs that are incurred wholly and exclusively for your work are often allowable. That commonly includes professional body subscriptions, medical indemnity insurance, mandatory continuing professional development, specialist equipment and mileage between sites. The position is less clear for general training that opens up a new area of practice rather than maintaining existing skills, so it is worth checking before a claim is made.
Medical care provided by appropriately registered professionals is generally exempt from VAT, which is why most consultations and treatments are not subject to it. The position changes for services that are not medical treatment — cosmetic and aesthetic procedures provided for appearance rather than health reasons are typically standard-rated, as are retail sales from a pharmacy or clinic shop. Registration thresholds and partial exemption can both become relevant, so the treatment should be reviewed rather than assumed.
Yes. We can run payroll for salaried clinicians, nurses, reception and administrative staff, and for locums engaged on an employment basis. That includes calculating PAYE and National Insurance, issuing payslips, handling starters and leavers, producing RTI submissions and dealing with pension auto-enrolment duties. Where a locum works through their own limited company, the engagement needs to be looked at separately, as the tax treatment is different.
HMRC expects individuals to keep records supporting their tax return for at least five years after the 31 January filing deadline for that tax year, and limited companies to keep accounting records for six years from the end of the relevant accounting period. In practice, keeping invoices, bank statements, mileage logs, pension statements and expense receipts organised by month makes the year-end process far quicker and reduces the risk of an allowable expense being missed.
