In practice, that usually means bookkeeping, Self Assessment for sole traders and partners, annual accounts and Corporation Tax for limited companies, VAT returns where registration applies, payroll for practice staff and pension auto-enrolment administration. What makes it specialist is the treatment of sectorspecific issues, such as NHS contract income, private patient fees, locum earnings, exempt versus taxable services for VAT, and expenses like indemnity insurance, professional registration and clinical equipment.
It often is, because the tax treatment of healthcare income is not always obvious. A locum working across several practices may have multiple payers and inconsistent monthly income, which affects how Self Assessment payments on account work. A dentist may have a mix of NHS and private treatment, plus laboratory costs and staff wages. A specialist accountant is more likely to spot missed expenses, question an incorrect VAT treatment or flag a structure that no longer fits, which a general practitioner accountant may not think to review.
Yes. Combining NHS and private income is common, and the main task is making sure every source is captured accurately and reported on the correct basis. NHS income may arrive through contracts, session payments or pension-related arrangements, while private fees might be invoiced directly or through a practice. The records need to reconcile across all of them, and expenses must be apportioned sensibly where they relate to both. Where relevant, we also consider how the total income affects payments on account and any higher rate liability.
Many medical and healthcare services are exempt from VAT, but exemption is not automatic across everything you do. Income from cosmetic procedures, retail sales such as pharmacy products outside prescriptions, some occupational health services and non-clinical consultancy can be standard-rated. If you have a mixture of exempt and taxable income, you may be required to register and account for VAT, and a partial exemption calculation may be needed to work out how much input VAT you can recover. This is an area where getting the treatment wrong can be costly in both directions.
Broadly, expenses must be incurred wholly and exclusively for your work. For healthcare professionals, that commonly includes professional indemnity insurance, registration and licence fees, professional body subscriptions, continuing professional development and mandatory training, specialist equipment and software, clinical clothing that is not ordinary everyday wear, professional journals, and a proportionate share of home costs where you genuinely run administrative work from home. Motor and travel costs can also be claimable for business journeys, though ordinary commuting is not. Records matter, so receipts and a clear note of the business purpose should be kept.
Yes. Payroll support can cover salaried clinicians, nurses, receptionists, practice managers, part-time and bank staff, as well as the pension auto-enrolment duties that apply to eligible employees. That includes calculating pay and deductions, issuing payslips, submitting real time information to HMRC and handling starters and leavers correctly. Practices with a mix of permanent staff and locum payments often find that separating these properly avoids confusion in both payroll and the accounts.
