In practice that means Self Assessment returns that combine salary, NHS pension figures, private fees, locum income and practice profits; annual accounts and Corporation Tax returns for incorporated practices, clinics and healthcare businesses; bookkeeping and VAT returns where registration applies; payroll for practice staff and associates, including pension auto-enrolment; and tax planning on areas such as business structure, equipment purchases and how you take money out of the practice. The emphasis is on reporting your income correctly rather than squeezing it into a standard template.
Both parts need to be reported, but they are treated differently. Salaried NHS income usually arrives through PAYE, while private fees, locum sessions and any practice profit share generally need to be included in your Self Assessment return. NHS pension contributions and annual allowance considerations can also affect your position. The important thing is that everything is captured in one coherent return, with the right expenses offset against the right income streams, so you are not paying tax on figures that should have been reduced or missing relief you are entitled to.
Many medical and healthcare services are exempt from VAT, but exemption is not automatic across everything a practice does. Services such as cosmetic or aesthetic treatments, dispensing, retail products, medico-legal reports and some consultancy work may fall outside the exemption. If you provide a mix of exempt and taxable services, turnover thresholds and partial exemption rules can become relevant. The sensible approach is to review what you actually supply and charge before assuming you are outside the VAT system entirely — or before registering when you may not need to.
