The work usually combines compliance and advice. On the compliance side that means preparing and filing Self Assessment returns for self-employed clinicians and locums, annual accounts for sole traders, partnerships and companies, Corporation Tax computations, VAT returns where relevant, and payroll for clinical and support staff. On the advisory side it includes reviewing whether your current structure still suits how you work, identifying allowable expenses you may be overlooking, planning the timing of income and purchases, and answering questions when a new contract or working arrangement raises a tax issue. Many clients also want bookkeeping kept up to date so that returns and accounts are based on accurate records rather than estimates.
If you receive income that has not been taxed at source, such as locum fees, private consultation income, sessional payments or self-employed therapy work, you will normally need to register with HMRC and file a Self Assessment return each year. That applies even where you also hold an NHS post with tax deducted through PAYE, because the untaxed income still has to be reported. The return needs to show gross income and the expenses you are entitled to claim, and it is worth keeping records of both throughout the year rather than reconstructing them at the deadline.
NHS salary is normally taxed through PAYE, while private fees, insurance-funded work and self-employed sessions are reported through Self Assessment. Both need to be reflected accurately on the same return, and the interaction between them can affect how much tax is due overall. Where you also have partnership income from a practice, there may be a partnership return in addition to your personal one. Getting this combination right is one of the main reasons healthcare clients prefer an accountant familiar with mixed income rather than treating each stream in isolation.
Common claims include professional registration and revalidation costs, indemnity insurance, specialist equipment and its maintenance, clinical software and subscriptions, professional journals, uniforms and laundry where applicable, and travel between separate work locations. Training that maintains existing skills is generally treated differently from training that qualifies you for a new area of work, and commuting between home and a regular workplace is not usually allowable. Mixed-use costs, such as a room at home used for administration, need to be apportioned on a reasonable basis. Records should support every claim you make.
It depends on what you supply. Many medical and healthcare services are exempt from VAT, but other income — including retail sales, dispensing, cosmetic treatments, some veterinary services and certain consultancy — is standard-rated or treated differently. A business with a mixture of exempt and taxable turnover needs to monitor its position carefully, because the VAT registration threshold applies to taxable turnover and can be reached by a side of the business you had not considered. Once registered, partial exemption rules may also affect how much input VAT you can recover.
