He helped me sort out a very complicated backlog of accounts and prepared my overdue tax returns with professionalism and attention to detail. Throughout the process, he was incredibly patient, took the time to answer all of my questions, and explained everything clearly. He guided me through each step and helped rectify issues that had built up over time, making what felt like an overwhelming situation much more manageable. We worked with a few different accountants before finally settling with Accotax, and we’re really glad we did. Their team is great. Abdul is responsive and easy to reach whenever I have a question, which makes a big difference. They take the time to understand how our business works and what our needs are. They are very knowledgeable, and when they don’t have the answer straight away, they take the time to look into it and get back to us quickly. Anum, who looks after our account on a day-to-day basis, including reconciling invoices and preparing the annual accounts, is amazing. She is professional, efficient, and follows the processes we have put in place together. She's also very quickin getting back to us whenever needed. We’re very happy to work with her and hope to continue doing so for as long as possible. Overall, Accotax has made our accounting experience much smoother and easier. We see them as a long-term partner as our business continues to grow. I would 100% recommend them.Posted on Google![]()

The core compliance work is similar, but the detail differs considerably. A healthcare-focused accountant understands how NHS income, private fees, locum payments and other clinical earnings are reported, which professional costs are allowable, how VAT applies to exempt and standard-rated healthcare services, and how payroll works for a mix of salaried staff, associates and locums. They also recognise the practical pressures of clinical work, such as irregular payment cycles and limited time for record keeping, and design the service around those realities rather than expecting you to fit a standard small-business template.
Many self-employed clinicians and locums do need to file a Self Assessment return, and you must notify HMRC that you have taxable income by 5 October following the end of the relevant tax year. Where tax has already been deducted at source from some engagements, that needs to be reported correctly so you are not taxed twice. The safest approach is to review all your income sources together, confirm which are self-employed and which are employment, and make sure your return reflects each one properly.
Typical allowable expenses include professional registration and revalidation costs, indemnity or professional liability insurance, membership of professional bodies, specialist equipment and uniforms, business mileage, and the costs of running a practice such as premises, software and administrative support. Expenses must be wholly and exclusively for your work, and mixed personal and business spending needs to be separated carefully. Capital purchases are generally treated differently from everyday costs, so it is worth checking before you buy rather than assuming a full deduction is available.
Many healthcare services provided by registered professionals are exempt from VAT, but not all. Private cosmetic treatments, some occupational health services, non-clinical goods and certain administrative or consultancy work can be standard-rated. A practice supplying both exempt and taxable services may need to apportion VAT and could be required to register once taxable turnover crosses the threshold. Getting the classification right from the outset avoids unexpected assessments, and a review of your supplies is usually a sensible starting point.
Yes. Practice payroll often covers a mixture of salaried employees, practice managers, reception and admin teams, and associates or locums engaged on different terms. Each arrangement carries different PAYE, National Insurance, pension auto-enrolment and reporting obligations, and off-payroll working rules may affect certain engagements. Running payroll accurately means applying the correct treatment to each individual, issuing payslips and reporting to HMRC on time.
You remain responsible for the accuracy of your tax position, so records do need to be kept. The good news is that they do not have to be complicated. Regular bank statements, income summaries, invoices and receipts for significant costs are usually sufficient, provided they are complete and retained for the required period. Keeping things reasonably current through the year, rather than assembling everything at the last minute, makes the process smoother and often reduces the tax you pay because nothing legitimate gets missed.
Yes. Much of the accounting process works well remotely, using secure document sharing and video or telephone calls for reviews and queries. That can be particularly useful for clinicians working shifts or splitting time between sites, since communication does not need to wait for a physical appointment. The key is agreeing a clear system for sending information, whether that is a shared cloud folder, accounting software access or periodic summaries from your bank and practice records.
It usually begins with a short conversation about your circumstances: your role, how you are structured, your income sources, how records are currently managed and what deadlines are approaching. From there, the position can be reviewed, any outstanding filings identified, and a suitable level of ongoing support agreed for bookkeeping, payroll, VAT, accounts or Self Assessment. If you are switching accountant, the transfer is normally straightforward and handled as part of the setup process.
