That means understanding the distinction between NHS and private earnings, the VAT exemption rules that apply to many clinical services, the expenses that are typically allowable for practitioners such as indemnity cover and professional registrations, and the payroll arrangements common in practices with associates and support staff. The technical rules are the same as anywhere else, but the context and the recurring problem areas are quite different.

Because payroll feeds directly into your accounts and tax position, handling it alongside bookkeeping tends to produce cleaner, more reliable figures overall.
Digital record keeping is now the standard approach for most businesses, and Making Tax Digital requires compatible digital records for VAT. That said, the underlying obligation is to keep accurate records of income and expenses, whether the original documents are paper or electronic. In practice, most healthcare clients find that scanning receipts and using software to categorise transactions saves considerable time and reduces the scramble at year end. What matters most is that records are complete, dated and can be produced if HMRC asks to see them, so the method should suit how you actually work.
