For most healthcare clients it combines a core set of compliance tasks with advice tailored to clinical income. On the compliance side that usually means bookkeeping, Self Assessment, annual accounts, Corporation Tax where you operate through a company, VAT returns where relevant, and payroll for practices that employ staff. On the advisory side it covers things like how to structure your income, what you can legitimately claim as a business expense, how to separate NHS and private work in your records, how to plan for a tax bill that changes with private earnings, and whether your current trading structure still suits what you do. The balance between the two depends on whether you are a single clinician or running a practice with a team.
Many self-employed clinicians can complete a Self Assessment return themselves, but the return is only straightforward if the right records exist behind it. The common problems are not the form itself — they are mixed income sources being merged, allowable expenses being missed or overclaimed, payments on account catching people out, and HMRC enquiries that are difficult to answer without a clean paper trail. An accountant's value here is often less about filing the return and more about making sure the figures are right, that nothing has been overlooked, and that you have someone to deal with HMRC on your behalf if questions arise.
We start by keeping each stream separately identifiable in your records, because the reporting and VAT treatment of different income types can differ. Salaried NHS or practice pay comes with its own documentation and tax already deducted, private fees need to be captured as they are invoiced and received, and locum income may arrive irregularly and be paid gross. Once these are tracked distinctly, the Self Assessment position becomes much clearer, payments on account can be estimated more accurately, and any VAT questions about private work or non-exempt services can be answered on the basis of real figures rather than assumptions.
Yes, and it is one of the areas where healthcare clients most often lose out. Professional registration and indemnity insurance, revalidation and continuing professional development, specialist equipment and clinical clothing, travel between sites, and a reasonable proportion of home or practice running costs can all be relevant where the link to your work is clear and supported by records. What matters is that the expense is genuinely incurred for the purposes of your work and that you can evidence it. We will review what you are claiming, flag anything that looks exposed, and make sure you are not overlooking legitimate costs simply because nobody has told you they count.
Not every healthcare business does, and the answer depends on the mix and value of what you supply. Many healthcare services are exempt from VAT, but cosmetic treatments, retail products, non-healthcare income and certain other supplies are typically standard-rated. If your taxable income crosses the registration threshold, registration becomes compulsory, and voluntary registration can occasionally be worth considering. For businesses with a combination of exempt and taxable supplies, partial exemption rules can apply, which affect how much input VAT you can recover. This is a technical area where getting the treatment wrong in either direction creates problems, so it is worth reviewing properly rather than guessing.
Practice payroll usually involves several different arrangements at once — salaried clinicians, associates, part-time reception staff, and possibly partners taking drawings rather than a salary. Each has different PAYE and National Insurance treatment, and pension auto-enrolment duties apply to eligible staff. We run the full payroll cycle, including payslips, RTI submissions to HMRC, pension contributions and year-end reporting, and we coordinate with the practice on starters, leavers, changes in hours and any irregular payments. Handling this in one place avoids the mismatch that commonly arises when payroll is done separately from the practice accounts.

Most of the work can be handled without you needing to visit an office, using secure document sharing, accounting software access and calls or video meetings at times that fit around clinics, surgeries and patient lists. What we need from you depends on the services in place, but as a starting point it is usually bank statements, income records for each stream, details of expenses with supporting receipts, payroll information for any employees, and copies of any previous accounts, tax returns or HMRC correspondence. If your records are incomplete, that is not a barrier — we can start from where you are and work forwards.
