Healthcare accounting covers the same core areas as any business accounting — bookkeeping, annual accounts, tax returns, payroll and VAT where relevant — but applied to the specific income and cost patterns of clinical work. That means correctly reporting NHS contract income, private patient fees, insurer payments and locum earnings, recognising expenses such as professional indemnity cover, registration fees, CPD and clinical equipment, and handling payroll for clinical and administrative staff. It also involves advising on business structure, profit extraction and cash flow, so the figures support decisions rather than just satisfying filing requirements.
They suit any healthcare professional or healthcare business with UK tax obligations. That includes GPs and GP partners, locum and private doctors, dentists and dental associates, pharmacists and pharmacy owners, nurses, physiotherapists and other allied health professionals, consultants running private clinics, and care providers or healthcare businesses that employ staff. It is equally relevant whether you operate as a sole trader, within a partnership, through a limited company or as part of a larger practice, since each structure brings different reporting and tax considerations.
Many healthcare professionals do, but it depends on your circumstances. Sole traders and partners will normally need to file a Self Assessment tax return. If you are employed but also receive private fees, locum income, rental income, significant dividend or savings income, or income from teaching or expert witness work, you may also need to file. Limited company directors are not automatically required to file a personal return simply by virtue of being a director, but other income sources often trigger the requirement. Certain costs, such as cosmetic or non-clinical activity, or expenditure with a personal benefit element, need more careful treatment, so records and context matter.
All taxable income needs to be reported, but different streams are often recorded separately so the figures are clear and defensible. NHS contract payments, private patient fees, insurer reimbursements and locum income may arrive at different times and in different formats, and each may carry different VAT treatment. In practice, the work involves reconciling those streams, matching them to the correct accounting period, and applying the right tax and VAT rules to each. Good bookkeeping routines from the outset make this far more straightforward than reconstructing records after the tax year ends.
Many healthcare services are exempt from VAT, but exemption is not automatic across everything a practice does. Retail sales, dispensary items, non-clinical products, cosmetic treatments and some mixed supplies can be standard-rated, which means a business may still need to monitor its taxable turnover against the registration threshold. Some healthcare businesses also choose to register voluntarily for practical reasons. We assess the nature of your supplies and your turnover position to determine whether registration is required and how VAT should be applied.
