They will also consider healthcare-specific issues such as the VAT exemption that applies to qualifying medical care, the correct treatment of associate or partner profit shares, pension contribution figures when planning tax, and the off-payroll rules that can affect locums engaged through agencies. The result is a return and a set of accounts that reflect your actual working pattern rather than a generic self-employed template.
Many locums are self-employed for tax purposes, which means registering with HMRC, filing a Self Assessment return each year and keeping evidence of business costs such as indemnity insurance, travel between sites, registration fees and equipment. It also means understanding payments on account, so you are not caught out by two tax bills close together. If you work through an agency or an intermediary, off-payroll working rules may apply and the position can be more complex. An accountant can confirm your status, handle the filings and make sure your records support the figures being reported.
VAT in healthcare is not a single answer. Medical care provided by a qualifying health professional is generally exempt, but other activities in the same business may be standard-rated — cosmetic or non-medical treatments, retail sales, room hire or training, for example. Businesses with a mixture of exempt and taxable income may need to apply partial exemption rules, which affect how much input VAT can be recovered. Registration also depends on your taxable turnover and the nature of your services, so it is worth reviewing the position before assuming you are outside the system.
Costs must be incurred wholly and exclusively for your work, but a useful range typically qualifies. That can include professional registration and indemnity insurance, CPD and mandatory training, professional body subscriptions, specialist equipment and instruments, uniforms and laundering where relevant, business mileage between sites, and a proportionate share of home running costs if you maintain a dedicated workspace. The key is keeping receipts and being able to explain the business purpose of each item if HMRC asks. Overclaiming creates risk, and underclaiming simply costs you money, so the list is worth reviewing each year.
Yes, and this is one of the most common situations in healthcare. Your NHS salary is taxed through PAYE, while private fees, locum sessions and similar income are reported through Self Assessment. Because tax is collected on the combined total, a shortfall can arise on the PAYE element, which often leads to payments on account for the following year. Planning around that is important, particularly when income fluctuates, and pension contribution figures can also affect the overall picture.
