The work covers the usual accounting functions, applied to healthcare circumstances. That typically includes bookkeeping and record keeping, preparing annual accounts, completing Self Assessment tax returns for sole traders, partners and locums, Corporation Tax computations for incorporated practices and clinics, payroll for clinical and administrative staff, VAT registration and returns where relevant, and tax planning around income levels and business structure. The difference is that each of these is handled with an understanding of how healthcare income and costs work — NHS pay and pension contributions, private fees, indemnity insurance, CPD and specialist equipment.
Both. A single locum GP, a sole practitioner physiotherapist or a part-time consultant with a small private caseload can benefit just as much as a multi-partner practice. For individuals, the focus is usually on accurate Self Assessment, identifying allowable expenses properly, and keeping records in a form that makes the return straightforward. For larger practices and clinics, the emphasis shifts towards partnership or company accounts, payroll, VAT, management information and supporting decisions on staffing and premises.
Not automatically, but many do. If you have untaxed income from private fees, locum work, medico-legal reports, teaching, rental income or significant savings and dividend income, you will usually need to file. Partners in a practice also need to report their share of profits, and company directors often need to file a return even where tax is collected through PAYE. If your only income is a salary taxed through PAYE and nothing else applies, you may not need to file — but it is worth checking rather than assuming.
NHS salary is normally taxed through PAYE, so it is already reported to HMRC, but it still needs to be included correctly in your return alongside anything else you earn. Private fees, locum payments and self-employed practice profits usually need to be reported with the associated expenses, such as indemnity cover, professional subscriptions and travel between sites. Getting the split right matters, because it affects the tax and National Insurance due, and because HMRC expects consistency between what has already been taxed and what you declare.
Common claims include professional registration and regulatory body fees, medical or professional indemnity insurance, CPD and required training, specialist equipment and clinical clothing, mileage for business travel between sites, and a reasonable proportion of home costs where a room is used for administration. The test is whether the cost is incurred wholly and exclusively for the purposes of your work, and whether you can evidence it. We review each client's circumstances rather than applying a blanket list, because what is claimable varies between professions and between employed, self-employed and incorporated arrangements.
It depends on the nature and level of your supplies. Many healthcare services are exempt from VAT, which means they do not count towards the registration threshold and no VAT is charged on them. However, other income — certain non-medical or cosmetic treatments, dispensing, retail sales, consultancy or equipment hire — may be standard-rated and does count. Where a business provides a mixture of exempt and taxable supplies, partial exemption rules can apply to the VAT you can recover on costs. This is an area where a review is usually worthwhile before assuming either that you must register or that you need not.
Yes. Payroll for healthcare employers often involves clinical staff, administrative staff, part-time roles and varying contracts, alongside pension contributions and pay adjustments. We can run payroll on a regular schedule, produce payslips, calculate PAYE and National Insurance, handle pension deductions, and prepare the reporting that HMRC requires. Taking payroll off the practice's to-do list also reduces the risk of errors that affect employees personally, which tends to be far more disruptive than correcting the figures in advance.
Remote working is entirely normal for accounting services. Documents and records can be shared securely, cloud accounting software lets bookkeeping, invoicing and reporting happen from wherever you work, and conversations can take place by phone or video call at times that fit around clinics and shifts. Many healthcare clients find this more practical than travelling to appointments, particularly locums working across multiple sites or practitioners with limited time between patient sessions. Whatever arrangement suits you, the same level of accuracy, deadline management and access to advice applies.
