The emphasis is on accounting that reflects how healthcare income genuinely works rather than forcing it into a generic format.
A specialist background helps considerably. Locum and sessional work often means multiple contracts, agency arrangements, expenses that travel between sites, and interaction with NHS pension rules. Getting the reporting right matters, particularly where income crosses tax bands or where the annual allowance is in play. Both sources are reported on the same return. Your NHS pay is usually taxed through PAYE, while private fees often arrive gross and need to be declared, with any tax due collected through the return. There is no universal answer. Sole trader status suits many clinicians with straightforward income and low administrative appetite, partnerships are common in general practice and shared clinics, and limited companies can offer tax efficiencies and limited liability in the right circumstances — though they bring additional filing and compliance obligations. The right choice depends on your income level, how profits need to be drawn, your plans for the practice and how much administration you want to take on.

That involves real-time information submissions to HMRC, payslips, statutory payments, auto-enrolment pension contributions and year-end reporting. For those in the NHS Pension Scheme, there are separate contribution and annual allowance factors worth reviewing alongside the payroll itself.
