The core work is much the same as any accountant — preparing accounts, completing tax returns, handling payroll and keeping records — but the detail is shaped by healthcare. That means understanding how NHS income, private fees, locum work, practice profits and partnership shares are taxed, how clinical expenses such as indemnity cover and regulatory fees should be treated, and how healthcare employers handle payroll and pensions. It also means advising on the structure that suits your circumstances, whether that is a sole trader arrangement, a partnership or a limited company.
It is suitable for most people earning a living in healthcare. That includes GPs and GP partners, hospital doctors and consultants, dentists and dental practice owners, pharmacists, nurses and advanced practitioners, physiotherapists and other allied health professionals, psychologists and therapists, locums and sessional clinicians, and the care providers, clinics and medical practices that employ them. If your income, expenses or employment arrangements involve healthcare in some form, the accounting considerations are usually quite specific.
Not always, but often. If you are employed with tax deducted through PAYE and have no other income, you may not need to file. However, a Self Assessment return is generally required if you have self-employed income such as private fees or locum work, receive income from a partnership, have untaxed income from investments or property, or have income that HMRC has not already taxed correctly. If you are unsure, it is worth checking before the filing deadline rather than after it.
NHS salary is usually taxed through PAYE as employment income, while private practice fees and many locum arrangements are treated as self-employed income, reported through Self Assessment with Class 2 and Class 4 National Insurance potentially applying. Where both exist, the two need to be reported correctly and kept separate in your records. Practice profits in a partnership are handled differently again. Getting the classification right at the outset avoids corrections later.
Generally, expenses must be wholly and exclusively incurred for your work. Common examples include professional indemnity insurance, registration and regulatory body fees, medical equipment and specialist clothing, professional journals and subscriptions, and a reasonable proportion of costs such as home office use, phone and travel between work locations. Some items are more straightforward than others, and claiming personal costs as business expenses is a common pitfall. We can help you judge what is defensible.
Many healthcare services are exempt from VAT, which means they do not count towards the VAT registration threshold and VAT is not charged on them. However, not everything a practice or clinic does is exempt — cosmetic treatments, some products, retail sales in a pharmacy and certain non-medical services can be standard-rated. If you have a mix of exempt and taxable income, registration and partial exemption rules can apply. It is worth reviewing your position rather than assuming healthcare is always exempt.

Payroll for healthcare employers often involves salaried clinicians, part-time and bank staff, locum payments and administrative teams, along with pension auto-enrolment obligations and the reporting that goes with them. Yes. Much of the accounting relationship can be handled remotely using secure digital records and document sharing, which suits clinicians who work irregular hours or split their time between sites. Records can be sent electronically, questions can be answered by email or phone, and meetings can take place online.
