The exact mix depends on whether you work as a sole trader, a partner or through a limited company.
Yes. Locum work, part-time contracts, bank shifts and private sessions often create several small income streams that still need to be reported accurately. If you are self-employed for some of your work, you will usually need to file a Self Assessment return, and many clinicians in this position are also able to claim expenses such as mileage, indemnity cover, professional registration fees and CPD. If most of your income is taxed through PAYE but you also have private fees, a partnership share, rental income or dividends, professional advice can help you avoid overpaying or missing a filing requirement.
Not always. If all of your income is taxed through PAYE and there is nothing unusual about your tax affairs, you may not need to file. However, a return is often required if you also earn private fees, locum income, partnership profits, dividends from a practice company, or significant savings and investment income. A return may also be needed if you are affected by the high income child benefit charge or by pension-related tax charges. It is worth checking your position rather than assuming either way, because penalties for failing to notify HMRC can apply even when no tax is ultimately due.

VAT treatment in healthcare depends on what is actually being supplied, not simply on who is supplying it. Many medical and dental services provided by appropriately qualified professionals are exempt, but services such as cosmetic or aesthetic treatments with no medical purpose, non-medical therapies, retail sales of goods, and some dispensing arrangements can fall outside that exemption and become standard-rated. Registration thresholds also matter, and voluntary registration sometimes makes sense even below the threshold. We review the specific services you provide and advise on how each should be treated, along with the record keeping needed to support that position.
Common claims include professional registration and indemnity insurance, professional body subscriptions, CPD and training relevant to your existing work, specialist equipment and clinical clothing, mileage between work locations, a proportionate share of home costs where you genuinely use space for administration, and accountancy fees. Items that are purely personal, or that only equip you for a new career, are generally not allowable. Because the detail matters and HMRC pays close attention to mixed personal and business costs, it is worth having your expenses reviewed rather than estimating them at the end of the tax year.
