The balance depends on whether you are a sole practitioner, an associate, a partner or a limited company.
Not by law, but the circumstances that make locum and associate work complicated are exactly where an accountant tends to add value. You may be paid gross by several practices, have expenses spread across different sites, need to judge whether your engagement should be treated as self-employment, and face payments on account that catch people out in the first profitable year. It depends on the nature and level of your income. Many medical and healthcare services are exempt from VAT, which means they do not count towards the registration threshold, but other activities such as cosmetic treatments, retail sales, room hire or non-medical consultancy may be standard-rated and do count. Where you provide a mix, the treatment has to be applied correctly to each income stream, because assuming everything is exempt, or everything is taxable, can lead to an incorrect return.
There is no single answer that suits every healthcare professional. Sole trader status is straightforward and works well for many locums and individual practitioners, partnerships remain common in general practice and dental practices, and a limited company can be appropriate where profits are retained or where you want a clearer separation between personal and business finances. The right choice depends on your income level, pension position, plans for the practice and how much administrative complexity you are comfortable with.
