The work covers the same core areas as any accountancy service, but applied to the way healthcare income and costs behave. That includes preparing annual accounts and tax returns, maintaining bookkeeping, running payroll for staff and clinicians, managing VAT where relevant, advising on business structure, and dealing with HMRC correspondence. What makes it different is the detail: recognising which registration fees, indemnity premiums, equipment costs and travel expenses are allowable, understanding how NHS pay interacts with private fees and locum income, and knowing how partnership profit shares or practitioner companies are taxed.
There is no legal requirement, but the nature of locum and self-employed clinical work makes specialist support worthwhile. Income often comes from several hospitals, practices or agencies, each with different payment terms, and expenses such as travel, accommodation, professional registration, indemnity cover and CPD need to be captured consistently. If you work through your own limited company, questions around contracts, IR35 status, salary and dividend planning and company filings add further complexity. A specialist accountant helps ensure those strands are reported correctly and that nothing is overlooked.
Both are taxable. NHS salary is usually taxed through PAYE, with a tax code applied by the employer, while private fees, locum earnings and self-employed clinical income are normally reported through Self Assessment. If you have both, you will generally need to complete a return, and the interaction between PAYE already deducted and tax due on the self-employed element is calculated there. Practice partners report their share of partnership profits instead, which affects how payments on account work and how much needs to be set aside during the year.
Common claims include professional indemnity insurance, annual registration fees for bodies such as the GMC, NMC, GDC or HCPC, professional body subscriptions, CPD and mandatory training, specialist equipment and clinical instruments, protective clothing, and travel between separate work sites. Ordinary commuting between home and a permanent workplace is not allowable. The key is evidence, so keeping receipts, mileage logs and bank records throughout the year makes the claim straightforward rather than a scramble at the deadline.
It depends on the value and type of supplies. Many medical and healthcare services are exempt from VAT, and exempt turnover does not count towards the compulsory registration threshold in the same way as standard-rated turnover. However, a pharmacy or clinic may also make standard-rated supplies, such as non-prescription retail sales, cosmetic treatments or non-medical products, and those do count. Where a business makes both exempt and taxable supplies, partial exemption rules apply and voluntary registration can occasionally be beneficial. This is an area where specific advice on your own figures matters more than a general rule of thumb.
Yes. Payroll support can cover the full practice team, including reception and administrative staff, nurses, practice managers, associate dentists or clinicians, and company directors. That means calculating pay, PAYE and National Insurance, submitting real time information to HMRC, producing payslips, handling starters and leavers, issuing P60s at year end, and administering pension auto-enrolment duties. Where a clinician receives a mix of salary and dividends, we also coordinate the salary element with the wider tax position so the two are not planned in isolation.
As a partner, you are taxed on your share of the partnership's profits through Self Assessment rather than through PAYE, which means you are responsible for payments on account and for Class 4 National Insurance. Practice accounts need to be prepared and agreed so each partner's share is accurate, and timing matters because profits taxed in one year are often based on an earlier accounting period. Pension contributions, including the interaction between practice profits and NHS pension arrangements, can also affect the overall position, so it is worth reviewing each year rather than leaving it to the filing deadline.
