VAT treatment varies between exempt medical services and standard-rated supplies, and payroll often involves a mix of employed staff and self-employed clinicians. An accountant familiar with those patterns is less likely to misclassify something or miss a claim.
You are not obliged to use one, but the returns become considerably more involved once you have more than one income source. Locum work through agencies, private consultations, and any employed role all need to be combined correctly, with expenses apportioned sensibly. Payments on account can also create unexpected cash flow pressure in the second year of self-employment. An accountant helps ensure the return is accurate and that you have a realistic idea of what is due and when.
Both are generally taxable, but they arrive in different forms and need to be recorded correctly. It depends on what you supply and how much standard-rated income you generate. Medical treatment provided by appropriately registered professionals and NHS dispensing are commonly exempt, but cosmetic treatments, retail sales, some non-medical services and certain equipment sales are standard-rated. Once standard-rated turnover crosses the registration threshold, registration is generally required. Mixed practices need to apportion income correctly rather than assuming everything is exempt.
Yes. Payroll for clinical, reception and administrative employees covers payslips, RTI reporting, statutory payments and pension auto-enrolment duties. Self-employed clinicians such as locums or associates are not run through payroll in the same way, and their status needs to be correct. Getting that distinction right matters, because treating someone as self-employed when they should be employed can create problems further down the line.
Remote working is entirely practical for healthcare clients. Records can be shared securely, payroll information submitted electronically, and meetings held by phone or video at times that fit around clinics and surgeries. Many practitioners find this easier than arranging cover for an appointment. If you prefer to meet in person at key points, that can usually be arranged as well.
It helps to have a rough picture of your income sources, how you are currently structured, any staff you employ, and whether you are registered for VAT.
