The work covers the accounting and tax obligations that come with clinical practice. For individuals, that usually means Self Assessment covering NHS income, private fees, locum work and any other earnings, along with advice on allowable expenses and payments on account. For practices and clinics, it typically includes annual accounts, Corporation Tax or partnership returns, bookkeeping, payroll for clinical and support staff, VAT returns, management accounts and tax planning. The focus is on getting the compliance right while also making the numbers useful for decisions about the practice.
It suits most people and organisations earning from healthcare. That includes GPs and GP partners, consultants and hospital doctors with private practice income, dentists and dental associates, pharmacists and pharmacy owners, nurses and nurse practitioners, physiotherapists and other allied health professionals, psychologists, locum clinicians, care providers, and the owners of private clinics or multi-disciplinary practices. If your income comes from clinical work, whether through the NHS, private patients, insurance work or a mixture, the tax and record-keeping issues are usually similar enough for specialist help to be worthwhile.
Both are taxable, but they behave differently in practice. NHS income often arrives through contracts, salaried roles or pensionable employment, with PAYE already deducted, whereas private fees, locum shifts and medico-legal work are frequently paid gross and need to be declared and accounted for separately. If you have both, the Self Assessment return has to bring them together correctly, and getting that wrong can mean an unexpected tax bill, incorrect payments on account, or an enquiry. Part of our role is making sure each income stream is reported accurately and that any tax already paid is properly credited.

Travel between sites can also be claimable in certain circumstances. The key is that the expense must be wholly and exclusively for the work and supported by records. We help separate what is defensible from what is likely to be questioned, so claims are made with confidence rather than guesswork.
It depends on what you supply. Many healthcare services are exempt from VAT, but non-medical treatments, cosmetic procedures, retail sales, dispensing income, consultancy, room hire and some other activities can be taxable. If a business has a mixture of exempt and taxable income and crosses the registration threshold, partial exemption rules may apply, and the calculations need care. The right answer varies considerably between a GP practice, a dental practice, a pharmacy and an aesthetics clinic, so it is worth reviewing rather than assuming either that VAT does not apply or that it does.

Common triggers include working through several agencies, mixing NHS and private sessions, being caught by IR35-style rules, going over the VAT threshold, or receiving an unexpected payment on account demand.
