Different income streams are recorded and reported separately so the figures stay meaningful and the returns are accurate. NHS contract payments, private prescription income, retail sales and locum earnings each have their own characteristics, and how they are treated for tax purposes depends on whether you trade as a sole trader, partnership or limited company. We reconcile the income throughout the year, match it against the relevant costs, and make sure the reporting reflects what actually happened rather than an estimate. Where VAT enters the picture, the treatment of different income types is considered individually.
It depends on your situation. If you are self-employed or work as a locum, you will normally need to file a Self Assessment return. Pharmacists who are employed but have additional untaxed income — locum work, rental income, consultancy or savings above certain thresholds — may also need to file. If you are employed only, with tax fully deducted through PAYE and no other income, you may not need to. Two pharmacists with similar roles can have different filing obligations, so it is worth checking rather than assuming.
Expenses that are wholly and exclusively incurred for your work are generally allowable, which for pharmacists often includes professional registration and indemnity insurance, uniforms and laundry, required publications, travel between work sites, and a reasonable proportion of home costs if you work from home. Some items are restricted or need apportioning, and the rules differ between employment and self-employment. Because the treatment depends on individual circumstances, we review your actual costs rather than applying a standard list, and we will tell you honestly where a claim is unlikely to be accepted.

Whether registration is required, and whether it is beneficial, depends on individual circumstances, so it is not something to assume either way. Where VAT applies, we can handle registration, prepare and submit returns, and advise on how the different income streams should be treated. If it does not apply to you, we will make that clear.
Switching is usually simpler than people expect.
