It depends on your circumstances. A doctor with a single salaried NHS post taxed through PAYE may only need help with a Self Assessment return if they have additional income or expenses to declare. However, once private fees, locum shifts, consultancy work, a practice partnership or a limited company enter the picture, the tax position becomes considerably more involved, and professional support usually saves both time and unnecessary overpayment.
Your NHS salary is normally taxed through PAYE, while private fees and self-employed income are reported on your Self Assessment return. The return brings both together, applies the correct tax bands across your total income, and accounts for tax already paid through PAYE. If private work has grown substantially, we may also consider whether payments on account apply and how best to plan for them.
Commonly claimed items include medical indemnity insurance, GMC and royal college or specialist society subscriptions, professional development and training relevant to your current role, equipment used for your work, business mileage and a proportion of home office costs where genuinely used for business purposes. What is allowable depends on your employment status and how the expense relates to your work, so it is worth checking rather than assuming either way.
Locum income is often treated as self-employment, though this depends on the terms of each engagement and how you are engaged. That means registering with HMRC, keeping records of fees and expenses, and reporting profits through Self Assessment. Some locums work through a limited company, which changes the compliance requirements considerably and may affect how tax is calculated and paid.
It can be beneficial in some situations, but it is not automatically the right answer. A limited company brings additional obligations such as annual accounts, confirmation statements and Corporation Tax returns, and the tax outcome depends on your total income, how much you draw from the company and your longer-term plans. We review your particular circumstances before making a recommendation.
Yes. Partnership accounts, profit share calculations and drawings are a regular part of the work we do for doctors in practice arrangements. Clear bookkeeping throughout the year helps each partner understand their share of profits and the tax that follows, rather than discovering the position only after the year end.
VAT is most often a consideration for doctors with private practice income, medico-legal work or consultancy services where turnover approaches or exceeds the registration threshold. Some healthcare services are exempt, so the position is not the same for everyone. We look at what you actually provide before advising on registration and returns.
