Not every doctor does, but many benefit from one. If all of your income is taxed through PAYE and you have no untaxed income, self-employment or complex tax affairs, you may not need much support beyond occasional advice. However, if you have private fees, locum income, consultancy earnings, rental income, investment income or a pension position that needs reviewing, you will usually need to file a Self Assessment return, and the way that return is prepared can affect how much tax you pay and how much risk you carry. Doctors in partnerships, those running a practice or company, and those whose income varies significantly from year to year often find professional support particularly worthwhile.
It depends on the type and level of your private income. Earnings from employment are usually taxed through PAYE, but untaxed income such as private fees, locum earnings or consultancy payments generally needs to be reported through Self Assessment, provided it exceeds HMRC's reporting threshold. Occasional small amounts may fall within HMRC's allowances, so it is worth checking your position rather than assuming either way. Costs that are private in nature, or that relate to ordinary commuting, generally do not qualify. Some expenses must be apportioned where there is a mixed business and private use. Both can work, and the better option depends on your level of private or locum income, how you are engaged, how you take money out, and your longer-term plans. Sole trader status is simpler administratively and may suit doctors with modest private income alongside NHS employment. A limited company can offer different tax and pension planning possibilities and may be attractive where income is substantial or where you are building a private practice or consultancy, but it brings additional compliance requirements, including annual accounts, confirmation statements and Corporation Tax filings.
