In practice that usually means preparing Self Assessment returns that combine NHS salary, private fees, locum payments or practice profits; producing annual accounts and Corporation Tax computations for incorporated practices; keeping bookkeeping up to date; running payroll and pension auto-enrolment for practice staff; and managing VAT returns where the business is registered. Alongside the compliance work, a healthcare accountant advises on structure, allowable expenses, capital allowances and cash flow so the financial side of the practice supports your wider goals rather than simply satisfying HMRC.
Doctors, GPs and consultants are a significant part of the picture, but healthcare accounting is not limited to medical practitioners. Dentists, pharmacists, nurses and advanced nurse practitioners, physiotherapists, podiatrists, dietitians, psychologists, optometrists and other allied health professionals all face similar accounting demands. So do care providers, private clinics and aesthetic practitioners. What matters is that the accountant understands how your income is structured and which tax and VAT rules apply to your particular service, rather than applying a generic business template.
If your only income is a salary taxed through PAYE and there is nothing else to declare, you may not need a return. Once you add private consultation fees, locum income, medico-legal work, rental of consulting rooms or any untaxed income, the position changes and a Self Assessment return is normally required. The return must account for both sources together, which is where errors tend to creep in. We prepare the return so that NHS income, private earnings and the expenses attached to each are reported correctly and consistently.
NHS salary is taxed through PAYE in the usual way, while private fees, locum payments and self-employed practice income are generally reported through Self Assessment, with tax and National Insurance settled separately. The distinction matters because the expenses you can claim differ between employment and self-employment, and because the timing of tax payments is different. Where practice profits or partnership shares are involved, the calculation becomes more involved again. Getting the split right avoids both overpaying and leaving yourself exposed to an enquiry.
Common claims include professional indemnity and malpractice insurance, annual registration and revalidation fees, mandatory continuing professional development, specialist equipment and instruments, clinical clothing and laundry where appropriate, professional subscriptions, practice or consulting room costs, and business mileage between sites. The test is that the expense is incurred wholly and exclusively for the work. Personal or dual-purpose costs, ordinary commuting and general living expenses do not qualify. We review your outgoings each year to make sure legitimate costs are captured and anything questionable is left out.
It depends on what you supply and how much you turn over. Many medical services are exempt from VAT, but cosmetic treatments, aesthetic procedures, non-medical services, retail sales of products and some consultancy can be standard-rated. A practice offering a mix of exempt and taxable services may need to register, and if it does, partial exemption rules often apply to the recovery of input VAT. Because the boundary is not always obvious, the sensible starting point is a review of your services and turnover before registration becomes compulsory, so there are no surprises.

For practices with associates, hygienists, receptionists or practice managers on varying hours and pay rates, keeping this running accurately on fixed deadlines is a significant administrative load, and it is one many practice owners are glad to hand over.
Yes. Accounting work is well suited to remote arrangements. Documents, records and reports can be shared securely online, meetings can be held by video or telephone, and questions can be answered by email. That said, we are happy to meet in person where it suits you. What matters most is having prompt access to your accountant when a decision needs making, whether that is about a new contract, a change in how you are paid, or a query from HMRC.
