I worked with Accotx on a HMRC reclaim and overall, they did ultimately deliver on their promise, which I appreciate. The process did take longer than any of us expected, partly due to external delays with the tax authorities, and partly because there were some hiccups in communication and follow-up. However, despite those challenges, they stayed professional and stuck to the original agreed terms. If you’re considering using them, be prepared to stay in regular contact to keep things moving along. They are absolutely capable, have the expertise and willing to work with you, and overall improving their services over time. I would certainly return for any future needs.

Hospitals and Hospital Organisations We Support

Hospitals come in many shapes and sizes, and the accounting support each one needs rarely looks the same. We work with private hospitals, independent hospitals, hospital operators and hospital groups, as well as smaller hospital businesses with a more focused range of services. Their accounting requirements can vary considerably depending on how they are structured, the activities they carry out, the income streams they rely on, and their staffing, premises, medical equipment, suppliers and day-to-day operating costs.

Private Hospitals & Day Surgery Units

Insurer and self-pay income reconciliation, consultant fee arrangements, statutory accounts and management reporting by department.

Care Homes & Nursing Homes

Occupancy and fee income tracking, local authority and private resident billing, agency staff cost control and CQC-ready records.

Specialist & Outpatient Clinics

VAT partial exemption on mixed medical and cosmetic work, capital allowances on theatre and imaging equipment, and payroll for clinical teams.

Multi-Site Healthcare Groups

Consolidated accounts, intercompany recharges, group VAT and site-by-site profitability reporting for expanding providers.

Our Accounting Services for Hospitals

Whether you need annual accounts prepared, day-to-day bookkeeping kept in order, management accounts that give you a clearer view of performance, or payroll and budgeting support, we can help. As Accountants for Hospitals, we shape our work around the realities of running a hospital, including staff costs, medical equipment, premises, suppliers, professional services and the different income streams a hospital may generate. We also provide financial reporting, along with Corporation Tax and VAT support where those apply to your organisation.

Corporation Tax & Capital Allowances

Full expensing and Annual Investment Allowance claims on theatre, imaging and ward equipment, plus group relief and tax planning.

Bookkeeping & VAT Partial Exemption

Healthcare is largely VAT exempt, but cosmetic work, pharmacy sales, catering and room hire are not. We calculate partial exemption correctly.

Statutory & Management Accounts

Companies House and HMRC filings on time, plus monthly management accounts showing income and margin by department or site.

Payroll for Clinical & Agency Staff

Large-volume payroll including bank and agency shifts, unsocial hours, statutory pay and pension auto-enrolment, filed accurately every cycle.

NHS Pension & Consultant Payments

NHS Pension administration and support for consultants on Annual Allowance, plus correct treatment of self-employed consultant fees.

Key Accounting Challenges Hospitals Face

Running a hospital means balancing significant fixed costs against income that can arrive from several different directions, and that combination puts real pressure on cash flow and financial reporting. Payroll for clinical and non-clinical staff, medical equipment and capital expenditure, premises and facilities costs, and supplier payments all need to be planned for rather than simply recorded after the event. Corporation Tax and VAT can add further complexity, since the VAT treatment of hospital-related supplies depends on the nature of the supply, the activities involved, the organisation’s status and the specific circumstances.

FAQs About Accountants for Hospitals

They handle the accounting and tax work that sits behind a hospital's day-to-day operations. That typically includes bookkeeping, payroll for clinical and non-clinical staff, management accounts, annual accounts, financial reporting, budgeting and cash flow planning, along with Corporation Tax and VAT support where those apply. The aim is to keep your records accurate, your reporting useful and your compliance deadlines under control, while giving you a clearer picture of how the hospital is performing.

We support private hospitals, independent hospitals, hospital operators and hospital groups, along with smaller hospital businesses providing a more focused range of services. What they tend to have in common is a need for accounting support that understands the cost structure and income patterns of a hospital, rather than a one-size-fits-all service. If you are unsure whether your organisation fits, it is worth a conversation, because the right starting point is usually how you are structured and what you need from your accounts.

Pricing is based on the work involved, which depends on the size and structure of your hospital, the number of entities involved, the volume of transactions, your payroll and the level of reporting you need. We quote on a fixed, transparent basis so you know what to expect, and we avoid hidden fees. The clearest way to get a figure is to talk through your circumstances with us, so the scope is properly understood before anything is agreed.

Yes. Payroll is often one of the largest and most time-consuming areas of a hospital's finances, particularly where you have a mix of full-time, part-time and variable-hours staff across clinical and non-clinical roles. We can manage the payroll process, help with the associated reporting and employer obligations, and make sure the figures feed cleanly into your management accounts so staff costs are visible throughout the year rather than a surprise at the year end.

It depends on how your hospital is structured and what it does. Corporation Tax is relevant where a hospital operates through a limited company, and the treatment of different income streams and costs needs to be considered carefully. VAT is equally circumstance-dependent: the VAT treatment of hospital-related supplies varies with the nature of the supply, the activities involved, the organisation's status and the specific facts, so it should be reviewed rather than assumed in either direction.

Yes, and this is often where good accounting support earns its keep. Where income arrives from more than one source and costs are weighted towards staff, premises and equipment, management accounts help you see which activities are performing and where pressure is building. We can prepare regular management accounts, build cash flow forecasts around your actual payment patterns, and help you plan for large items such as equipment purchases rather than reacting to them after the fact.

We can. Equipment purchases and other capital expenditure tend to be significant and irregular, which makes them difficult to absorb if they have not been planned for. We help hospitals think through how a purchase affects cash flow and reporting, how it should be treated in the accounts, and how it fits alongside your existing commitments. That way, decisions about equipment, premises or facilities are made with a clear view of the financial consequences.

It usually starts with a conversation about your hospital, how it is structured and what you need from your accounting support. If you are switching accountants, we can talk through what is involved in moving across and what information we would need from you. From there we agree a scope of work that fits your circumstances, and once that is in place you have a clear point of contact for bookkeeping, payroll, reporting and tax matters as they arise.
