The work goes well past a single annual filing. It typically begins with keeping your records accurate throughout the year, reconciling bank accounts and patient fee income, and separating employment, locum, private practice and company income so the right figures reach the right return. From there it covers Self Assessment or Corporation Tax, payroll and pension reporting for staff, VAT returns where applicable, and management reporting that shows how the practice is performing. It also includes HMRC correspondence, responding to enquiries and planning ahead on areas such as structure, remuneration and major purchases.
Yes, and often for the same reasons practice owners do. Locum and associate work usually means several income sources, expenses that a general accountant may not recognise as allowable, and irregular monthly earnings that make payments on account difficult to predict. There are also indemnity insurance, registration, travel between sites and equipment costs to account for properly. Getting the classification of income right matters, particularly where agency payments, direct patient fees and salaried sessions sit alongside each other.
Allowable expenses are costs incurred wholly and exclusively for your work. For clinicians this commonly includes professional registration and membership fees, indemnity or malpractice insurance, clinical equipment and its upkeep, continuing professional development, specialist clothing such as scrubs or uniforms, and business travel including mileage between sites. Costs with a personal element, or those that relate to employed duties where expenses are claimed differently, need more careful treatment. The practical benefit is that identifying these properly reduces the profit figure your tax is calculated on, while keeping the claim defensible if HMRC asks questions.
An incorporated practice files annual accounts and a Corporation Tax return, and the company pays tax on its profits rather than you personally on the practice income. You then take money out through salary, dividends or a combination of both, and the split affects your personal tax, National Insurance and pension position. Director payroll obligations still apply even where you are the only employee, and benefits such as a company vehicle or private medical cover need reporting. Getting this balance reviewed alongside your personal circumstances is usually more valuable than copying whatever structure another practice uses.
Some healthcare services are exempt from VAT, while others, including certain cosmetic treatments, products, non-medical reports and some consultancy work, are standard-rated. If you supply a mixture, you may need to register, charge VAT on the standard-rated elements and use an apportionment or partial exemption method to work out how much input VAT you can recover. The detail matters, and incorrect treatment in either direction can be costly. We review what you actually supply and how it is invoiced before deciding how to handle registration and returns.
The simplest approach is to capture information as it happens rather than saving it for year end. Digital bookkeeping tools connected to your bank account, a consistent method for logging mileage and a habit of photographing receipts and storing them by month remove most of the pain. If admin is genuinely difficult to fit in, outsourcing the bookkeeping to us is often more cost-effective than losing evenings to it, and it means figures are available during the year for planning rather than only after the accounts are prepared.
