He helped me sort out a very complicated backlog of accounts and prepared my overdue tax returns with professionalism and attention to detail. Throughout the process, he was incredibly patient, took the time to answer all of my questions, and explained everything clearly. We worked with a few different accountants before finally settling with Accotax, and we’re really glad we did. Their team is great. Abdul is responsive and easy to reach whenever I have a question, which makes a big difference. They take the time to understand how our business works and what our needs are. They are very knowledgeable, and when they don’t have the answer straight away, they take the time to look into it and get back to us quickly. Anum, who looks after our account on a day-to-day basis, including reconciling invoices and preparing the annual accounts, is amazing. She is professional, efficient, and follows the processes we have put in place together. She's also very quickin getting back to us whenever needed. We’re very happy to work with her and hope to continue doing so for as long as possible. Overall, Accotax has made our accounting experience much smoother and easier. We see them as a long-term partner as our business continues to grow. I would 100% recommend them.Posted on Google![]()

The core compliance work is similar — accounts, tax returns, payroll and VAT. What differs is the context. Healthcare clients typically have income from several sources such as NHS practice payments, private fees, locum sessions, associate income, teaching or medico-legal reports, and each needs to be captured correctly. There are also healthcare-specific considerations around pension contributions and annual allowance charges, the VAT exemption for medical care, professional indemnity and registration costs, and the practical reality that clinical time leaves little room for administration. Specialist healthcare accountants work with those issues as a matter of routine.
They suit anyone earning income from healthcare work: GPs and GP partners, hospital doctors and consultants, locum doctors, dentists, dental associates and practice owners, pharmacists and pharmacy businesses, nurses and nurse practitioners, allied health professionals such as physiotherapists and podiatrists, psychologists and therapists, carers and care home providers, and the owners of clinics or private medical practices. They also suit healthcare professionals who are employed but have private, teaching or consultancy income that needs to be reported through Self Assessment.
Both are generally taxable, and they are reported together on your tax return, but they are treated differently for record keeping and sometimes for VAT. NHS income usually arrives through practice or payroll arrangements, while private fees, locum payments and medico-legal work often come as untaxed income that you must declare. If you are a partner in a practice, your profit share is reported through the partnership return as well as on your own return. Keeping the streams distinguishable throughout the year makes the return far easier to prepare and reduces the risk of under- or over-declaring.
Broadly, costs incurred wholly and exclusively for your healthcare work. These commonly include professional registration fees, professional indemnity insurance, continuing professional development and required training, specialist equipment and its maintenance, professional subscriptions, and business mileage or travel between work sites. Ordinary commuting between home and a permanent workplace is not usually allowable. Use of home for administration, phone and internet costs, and a proportion of certain running costs may also be claimable where the business use can be evidenced. Records need to support every claim, so keeping receipts and a mileage log makes a real difference.
Most medical and healthcare services are exempt from VAT, so registration is not always required even at significant turnover. However, VAT becomes relevant where you make taxable supplies — for example dispensing medicines and retail products, some cosmetic or non-medical treatments, or the sale of goods alongside services. If exempt and taxable supplies are mixed, partial exemption rules apply, which affect how much input VAT can be recovered. If you are approaching the registration threshold or already run a mixed clinic, it is worth reviewing your position before the question becomes urgent.
Yes. Payroll for healthcare employers usually involves a mix of salaried clinical staff, part-time reception and administrative teams, and associates or locums paid on varying patterns. Each arrangement brings its own tax code, pension and reporting considerations, and payslips must be produced accurately on schedule. We can manage routine payroll processing, real-time reporting, auto-enrolment pension administration and the year-end submissions that follow, so staff are paid correctly and practice obligations are met without the paperwork landing on a clinician's desk.
It depends on your circumstances rather than a single 'right' answer. Sole trader status is straightforward and suits many locums and single-handed practitioners. Partnerships are common in general practice and dental or pharmacy ownership, with profit shares and partnership tax returns. A limited company can be appropriate for some healthcare businesses, but it brings additional filing, payroll and compliance duties and does not suit every type of income or contract. The decision should factor in your income level, profit extraction needs, pension position and future plans, and it is worth revisiting as your work changes.
It begins with a conversation about how you work, your income sources and what is currently causing difficulty. From there we review your accounting and tax position, confirm which services you need — bookkeeping, Self Assessment, payroll, VAT or company accounts — and agree a timetable for information. Much of the process can be handled remotely using secure document sharing and digital records, which suits practitioners who work across different sites or keep limited office hours. Where meeting in person in Nottingham is more practical, that can also be arranged.
