Not every doctor does. If your income is entirely from salaried NHS employment and is taxed through PAYE, you may not need to file a Self Assessment return at all. The position usually changes once private fees, locum income, consultancy work, partnership profits or untaxed income are involved, or where you have higher income that creates additional reporting obligations. An accountant can help you work out whether a return is required and, if it is, make sure it is prepared correctly.
Your NHS employment income is normally taxed through PAYE, but it still needs to be included on your Self Assessment return alongside any private medical income, locum earnings or consultancy fees. The two are reported together and your tax is calculated on the combined total, with credit given for tax already deducted through PAYE. This is one of the main reasons doctors find their returns more involved than a standard employment tax position.
Commonly claimed items include medical indemnity and professional indemnity insurance, professional body and registration subscriptions, approved training and continuing professional development costs, specialist equipment and reference materials, and travel undertaken for work purposes rather than ordinary commuting. Whether a specific cost is allowable depends on your employment status, how the expense relates to your work and the structure you operate through, so it is worth checking individual items rather than assuming a blanket rule applies.
Locum work can be paid through PAYE, through an agency, or directly to you as self-employed income, and the treatment follows how the engagement is actually structured. Where you are genuinely self-employed, you would typically report income and expenses through Self Assessment, which also allows allowable costs to be claimed. Where you operate through a limited company, different rules apply. Because locum arrangements vary, the correct treatment should be based on your specific contracts rather than a general assumption.
It depends on your circumstances. Sole trader status is often adequate for small amounts of private or consultancy income, partnerships suit doctors running a practice together, and a limited company can offer a different approach for those with substantial private practice or consultancy earnings. Tax treatment, administration requirements, pension implications and how profits are extracted all differ between structures, so the right choice is a planning decision rather than a default one.
Only some do. VAT registration becomes compulsory once taxable turnover passes the registration threshold, and private medical services can be exempt from VAT in certain circumstances while other services are not. That means the question is rarely a simple yes or no. If you provide private fees, clinic services or consultancy work, it is worth reviewing whether registration applies to your activities and what it would mean for your pricing and record keeping.
