He helped me sort out a very complicated backlog of accounts and prepared my overdue tax returns with professionalism and attention to detail. Throughout the process, he was incredibly patient, took the time to answer all of my questions, and explained everything clearly. We worked with a few different accountants before finally settling with Accotax, and we’re really glad we did. Their team is great. Abdul is responsive and easy to reach whenever I have a question, which makes a big difference. They take the time to understand how our business works and what our needs are. They are very knowledgeable, and when they don’t have the answer straight away, they take the time to look into it and get back to us quickly. Anum, who looks after our account on a day-to-day basis, including reconciling invoices and preparing the annual accounts, is amazing. She is professional, efficient, and follows the processes we have put in place together. She's also very quickin getting back to us whenever needed. We’re very happy to work with her and hope to continue doing so for as long as possible. Overall, Accotax has made our accounting experience much smoother and easier. We see them as a long-term partner as our business continues to grow. I would 100% recommend them.Posted on Google![]()

The work combines day-to-day accountancy with an understanding of how healthcare income behaves. Typically that means preparing and filing your Self Assessment return or annual accounts, keeping bookkeeping up to date, running payroll, handling VAT where your activities are not wholly exempt, and advising on how NHS pay, private fees or practice profits are taxed. It also means spotting issues early — for example where pension growth, a change of contract or a new private clinic could affect your position — so you are not reacting to a bill after the event.
Yes. Combining PAYE income from an NHS role with self-employed earnings from private practice or locum work is one of the most common situations we deal with. The NHS income is taxed through PAYE but still has to be reported on your tax return, while the self-employed side needs its own records, expense claims and payments on account. Handling the two together properly helps avoid paying too much or too little during the year and reduces the chance of HMRC asking questions later.
Off-payroll working rules are a key consideration, because many NHS engagements fall within the public sector, and the responsibility for determining employment status can sit with the engager rather than with you. Alongside that come the usual limited company duties: annual accounts and a confirmation statement, Corporation Tax, PAYE if you draw a salary, VAT once turnover passes the threshold, and properly documented dividends. It is far easier to review the structure before a contract is signed than to unpick it afterwards.
Broadly, costs that are wholly and exclusively for your work and not reimbursed by an employer. Common examples include professional registration and indemnity insurance, mandatory training and CPD, specialist equipment, clinical clothing, professional body subscriptions, and a proportionate share of home running costs where you do admin there. Travel between sites or to temporary workplaces may qualify, though ordinary commuting normally does not. Evidence matters — HMRC expects records rather than estimates, so keeping receipts and mileage logs as you go is worthwhile.
It depends on the nature of the work. Many medical services provided by registered health professionals are exempt, which means you do not charge VAT on them — but it also generally means you cannot reclaim VAT on related costs. If turnover from taxable activities such as cosmetic or aesthetic treatments, medico-legal work, retail sales or certain non-medical services goes above the registration threshold, registration may be required. Where a business has both exempt and taxable income, partial exemption rules come into play, so it is worth reviewing the position before the threshold is reached rather than after.
Practices often pay several different groups at once: reception and admin staff on PAYE, salaried clinicians, and self-employed associates or locums who invoice the practice. Each needs different treatment. PAYE brings real-time reporting, payslips and pension auto-enrolment duties, while self-employed clinicians are normally paid gross and need a clear record of their invoices. Getting the classification right matters, since it is a common area of HMRC interest, so contracts and working arrangements are worth reviewing periodically.
Yes. Most of what we need moves electronically — bank statements, invoices, payroll data, pension figures and other records — and secure online tools make it straightforward to share documents and review numbers without meeting in person. Many healthcare clients prefer this because shift patterns and clinic hours make daytime appointments difficult. What matters more than the format is having a clear point of contact and an agreed timetable for bookkeeping, payroll and filing.
It usually begins with a conversation about your circumstances: how you are structured, where your income comes from, what accounting is already in place and what is causing you concern. From there we review your current position, confirm what needs to be filed and by when, and set out the level of support that fits your situation. If you already have an accountant, the handover is normally a straightforward matter of contacting them for the relevant records and professional clearance.
