Not every doctor does. If you have a single salaried NHS post, no untaxed income and straightforward affairs, your tax is largely handled through PAYE and an accountant may add limited value. The picture changes once there is anything additional: private consultation fees, locum income, medico-legal reports, consulting work, partnership profits, property income or a limited company. At that point, Self Assessment becomes more involved, expenses need to be identified correctly, and there are usually planning decisions worth making — which is where a specialist accountant earns their keep.
No. Whether you need to file depends on your income sources and circumstances. HMRC generally expects a return where there is untaxed income, such as private fees, locum earnings paid without tax deducted, consultancy income or partnership profits. Some doctors are also required to file because of high income, child benefit considerations or investment and property income. If you are unsure whether your situation requires a return, it is worth checking rather than assuming, because penalties for late filing apply even where no tax is ultimately due.
They are reported together on one Self Assessment tax return but taxed differently. NHS salary is taxed through PAYE during the year, while private fees, locum income and consultancy earnings are usually reported and taxed through Self Assessment. We reconcile the figures, make sure the PAYE already paid is credited correctly, account for NHS Pension Scheme contributions where relevant, and calculate what remains due. Where income is rising, we also look at payments on account so the following year's bill does not arrive as a surprise.
Commonly claimed items include medical indemnity and defence organisation subscriptions, GMC registration fees, professional body and royal college memberships, professional books and journals, equipment used for work, and travel to temporary or multiple work locations. Continuing professional development can also qualify where it maintains or updates skills you already use in your current role. Training that equips you for a new role or specialty generally does not. Home office costs and mileage have their own rules, and the detail matters — so the right answer depends on your individual circumstances.
Locum work often means income from several sources across the year, with travel between sites and periods where you may operate through a limited company or an umbrella arrangement. That brings expense records, contract status and sometimes VAT into play. If you contract through your own company, off-payroll working rules may apply depending on who the end client is, and that affects how you should be paid and taxed. These are areas worth reviewing early rather than discovering at the end of the tax year.
It depends. A limited company can suit some doctors with substantial private or consultancy income, particularly where profits are retained or extracted in a planned way. For others, sole trader or partnership treatment is simpler and no less tax efficient. Contract status rules, pension implications, the administrative cost of running a company and your wider income all need to be weighed up. There is no universal answer, and it is not a decision to make on the basis of a general rule of thumb.
Yes. Practice and partnership work involves profit shares, drawings, capital accounts, partnership tax returns and often payroll for staff. Private practices may also need to consider VAT, since some medical services are exempt while others are not, depending on what is being provided and by whom. Remote working is perfectly workable for doctors — documents can be shared securely, meetings held by video or phone, and much of the process handled digitally.
