In most cases, yes. If you work as a self-employed dentist or an associate dentist, you are generally treated as self-employed for tax purposes, which means you are responsible for declaring your income and expenses through Self Assessment. Where you also hold an employed role, or receive income from other sources such as property or investments, those may need to be included on the same return. We can review your arrangements and explain which income streams need to be reported and how to record them correctly.
NHS contract payments and private treatment fees are broadly both trading income for a self-employed dentist, but the way they are earned, invoiced and recorded can differ, and practice owners may receive income through a contract rather than on a per-patient basis. The tax treatment can also be affected by whether income is received personally, through a partnership or through a limited company. Because the detail matters, we look at your specific contracts and payment arrangements before advising on how each stream should be reported.
Dentists may be able to claim for costs that are incurred wholly and exclusively for the purposes of their work, which can include indemnity insurance, professional registration and subscriptions, continuing professional development, laboratory charges, materials, and a proportion of certain running costs where applicable. What qualifies depends on your working arrangement, so expenses that are allowable for a practice owner may not be treated the same way for an associate, and vice versa. We review claims carefully so that the position taken is defensible.
Some dental services are exempt from VAT, which means many practices do not need to register on the basis of their clinical income alone. However, the position can change where a practice also supplies goods or services that fall outside the exemption, such as certain cosmetic treatments, dental products or laboratory work. Whether registration is required depends on the nature and value of what the practice supplies, so it is worth reviewing your specific activities rather than assuming the exemption covers everything.
There is no single answer that suits every dentist. Associates often work as sole traders, principals frequently operate through partnerships or limited companies, and practice ownership can involve a mixture of structures. Each option affects how profits are taxed, how much administrative work is involved and how income can be extracted. A practice payroll typically covers calculating pay for dentists and team members such as hygienists, nurses, receptionists and practice managers, applying the correct tax codes, processing pension contributions where applicable, and reporting through Real Time Information. If the practice employs staff, payroll also needs to account for employment costs and any benefits provided.
