In many cases, yes. If you receive self-employed income — for example as an associate dentist paid a share of practice receipts, or as a sole trader running your own practice — you will usually need to report that income to HMRC through Self Assessment. The position can be different if you are employed and taxed through PAYE, or if you have a mixture of employed and self-employed income, which is common for dentists working across more than one practice. The tax return needs to reflect all your taxable income sources, and payments on account may also apply depending on your overall tax position.
Each arrangement can be treated slightly differently depending on the contract and whether you are engaged as self-employed rather than employed. In practice, this usually means keeping a clear record of income received from each source, tracking the costs connected to that work, and making sure your tax return reflects the correct totals. Where an expense relates to more than one practice, it helps to agree a sensible approach to apportionment and apply it consistently year on year, so the figures stand up if HMRC asks questions.
Expenses incurred wholly and exclusively for the purposes of your dental work may be allowable, which can include professional indemnity insurance, professional body subscriptions and registration fees, along with certain costs relating to uniforms, clinical equipment and continuing professional development. The rules are not automatic and depend on the nature of the expense and your working arrangements, so claims are best reviewed each year rather than assuming a particular cost is or is not allowable. It also matters whether you are employed or self-employed, as the treatment can differ.
VAT registration depends on taxable turnover and on the nature of the services provided. Some dental treatment may be exempt from VAT, while other services — such as certain cosmetic or purely elective treatments, or the sale of products — may be standard-rated. Because the balance of NHS, private and cosmetic work varies considerably between practices, your VAT position should be reviewed against your own circumstances rather than applied as a general rule, and it is worth revisiting if your service mix changes.
There is no single structure that suits every practice. Operating as a sole trader or partnership is common and relatively straightforward, while a limited company can offer different options for tax and profit extraction, alongside additional reporting obligations. The right choice often depends on profitability, how many principals or partners are involved, plans for expansion or eventual sale, and how you want to take money out of the business. It is usually sensible to review the structure periodically rather than setting it once and leaving it untouched.
At a minimum, practices generally need to keep records of income from all sources, including NHS contract payments, private treatment fees and any laboratory or product income, together with invoices, bank statements and receipts for expenses. Payroll records for employed staff, documentation for equipment purchases and details of any finance or lease agreements are also relevant. Keeping these records organised through the year makes annual accounts and tax returns considerably easier to prepare and reduces the risk of a last-minute scramble.
