Many associate dentists are treated as self-employed for tax purposes, which means they are responsible for registering with HMRC where required, filing a Self Assessment tax return and paying tax and National Insurance on their income. The tax treatment of each arrangement can vary, and it can also affect whether you are seen as employed or self-employed. Yes. Dental practices often employ a mix of nurses, receptionists, hygienists, therapists and administrative staff, and each can bring different payroll considerations, including PAYE, National Insurance, pension duties and statutory payments. We can handle payroll processing and the related reporting, and help you understand the total employment cost of your team rather than just gross pay. Where you use associates or locums on a self-employed basis, we can also advise on the record keeping that arrangement requires.
It depends. VAT registration is generally driven by turnover against the relevant threshold, and some dental services may be exempt while others are not, which can make the position less straightforward than it first appears — particularly where NHS work and private cosmetic or laboratory-related services sit side by side. If registration applies, there are different schemes and treatment options that may suit a practice. Because the answer varies with your mix of services and turnover, it is worth reviewing your position rather than assuming either way.
As a general guide, you should keep records of all income received, whether from NHS contracts, private fees or other sources, along with evidence of expenses such as indemnity cover, professional subscriptions, continuing professional development, clinical equipment, laboratory fees and travel between practices. Bank statements, invoices and receipts all help, and keeping NHS and private income clearly separated makes both bookkeeping and tax reporting far easier. If you operate through a limited company, company records will also be needed for annual accounts and Corporation Tax.
Not necessarily — it depends on your circumstances. Sole trader, partnership and limited company structures each carry different tax, reporting and administrative obligations, and what suits one dentist may not suit another, particularly where profit levels, drawings, pension contributions and plans to buy or sell a practice are involved. We can talk through how each structure might work for you and the practical implications, so the decision is made with proper information rather than assumption.
Switching is usually more straightforward than dentists expect. We would ask for details of your current arrangements and the records you hold, then liaise with your existing accountant to obtain the information needed for a clean handover, including any professional clearance requirements where relevant. If your previous records are incomplete or your bookkeeping has fallen behind, that can be sorted as part of the process. The aim is to move you across without disruption to deadlines or continuity of advice.
