Not every doctor does. If your only income is an NHS or hospital salary taxed through PAYE and you have no other income, your tax is usually handled by your employer and an accountant may not add much. The position changes once you have untaxed income such as private patient fees, locum sessions, consultancy, medico-legal work, teaching or rental income, because that income normally has to be reported through Self Assessment. Doctors with several income sources, fluctuating earnings, a practice partnership or a limited company generally find that professional support saves time and reduces the risk of errors, penalties and missed expenses.
Your NHS or hospital salary is normally taxed through PAYE, and your employer reports it to HMRC. Any private, locum or consultancy income that has not been taxed at source is then reported on a Self Assessment return, with allowable expenses deducted to arrive at the taxable profit. HMRC reconciles the two and collects the additional tax, often through an adjustment to your tax code or through payments on account if you are self-employed. Because the two systems interact, keeping clear records of private income and expenses throughout the year makes the return far more straightforward.
Commonly claimed costs include medical indemnity insurance, professional body and registration subscriptions, training and professional development that maintains or updates your existing skills, specialist equipment and textbooks, and travel or mileage for work-related journeys. Whether a particular cost is allowable depends on your employment status and on whether it was incurred wholly and exclusively for your work, and employed doctors face stricter rules than the self-employed. We review your expenditure as part of preparing the return and explain which items qualify.
Locum work can be structured in different ways. Some locum posts are employment, in which case tax may be deducted through PAYE, while genuinely self-employed locum work is reported on a Self Assessment return and taxed on profits after allowable expenses. The correct treatment depends on your contracts and working arrangements, not simply on the label used by the agency or trust. Self-employed locums also need to be aware of payments on account, which means tax is collected in advance based on the previous year's liability.
It can be worth considering, but it is not automatically beneficial. Incorporation may suit doctors with substantial private practice, consultancy or medico-legal income who want a different approach to profit extraction and pension planning, but it brings additional obligations such as statutory accounts, Corporation Tax returns, payroll and compliance with off-payroll working rules where they apply. NHS Pension Scheme considerations can also be relevant. We look at your income levels, plans and personal circumstances before recommending a structure.
Many medical services provided by registered health professionals fall within the medical exemption, but the exemption is not universal. Certain cosmetic procedures, medico-legal reporting and other non-medical services may be outside it, and whether you need to register depends on the level and type of your taxable income. If you provide a mix of exempt and standard-rated services, the rules around what you can reclaim can be more involved, so it is worth reviewing your position before your private work grows.
Keep invoices and payment records for private and locum work, bank and card statements covering your professional accounts, receipts for indemnity, subscriptions, equipment and training, a mileage log for practice and private work travel, pension statements, and your P60 or P45 from employed roles. If you use cloud accounting software, much of this can be captured as you go rather than stored in a folder for months. Good records not only support the tax return but also provide evidence if HMRC asks questions.
