A salaried doctor whose only income is taxed through PAYE may not need much beyond a check that their tax code is correct. Once you add any self-employed income, such as locum shifts, private practice fees, consultancy work, medico-legal reports or teaching, you enter Self Assessment and the picture becomes more involved. An accountant helps you report all of it accurately, claim the expenses you are entitled to, understand payments on account and avoid the amended returns that often follow a first attempt at doing it yourself. Whether that support is worthwhile depends on your circumstances, and we are happy to talk that through honestly.
Yes. NHS salary is usually taxed through PAYE, but it still needs to appear on your Self Assessment return alongside any private or self-employed income, and your tax code and payments on account need to reflect the combined total. We bring the sources together, make sure each is reported in the right place, and check that the resulting tax position is consistent. Where private income is significant or irregular, we also look at how payments on account will fall so the timing of tax bills is not a surprise.
Locum work is generally treated as self-employment, which means you report income and expenses on a Self Assessment return and pay tax and National Insurance through the self-assessment system. That also means you need proper records: timesheets, agency statements, mileage, equipment and any costs directly connected to the work. Many locums work through agencies and sometimes through umbrella arrangements, and the way you are paid affects what you need to report. We can help you work out your position and put a record-keeping routine in place that makes the return straightforward.
Commonly claimed items include medical indemnity insurance, GMC and royal college or specialist society subscriptions, professional development and training that maintains your existing skills, specialist equipment, and travel to temporary or temporary-elsewhere sites on a mileage basis. What qualifies depends on the nature of the expense and how it relates to your work, and some costs that seem obvious are not allowable. Rather than working from a generic list, we look at your actual costs and advise on each one, so claims are defensible if HMRC ever asks.
There is no single answer, because it depends on your level of private or self-employed income, the type of work you do, how much risk you carry and your wider financial plans. A limited company can suit some consultants and private practitioners, while many locums and doctors with modest additional income are better served as sole traders. If your private work sits close to these boundaries, it is worth reviewing rather than assuming, since getting it wrong can be costly to correct.
