Many doctors can prepare a simple return themselves, and there is nothing wrong with doing so. The question is whether your circumstances are still simple. Once private fees, locum income, consultancy or medico-legal work are added to NHS pay, the interaction between your tax code, payments on account and the balancing charge becomes harder to predict, and expenses have to be classified correctly. A specialist accountant is not essential for everyone, but it becomes valuable when your income sources multiply, your structure changes, or the cost of getting something wrong outweighs the cost of the support.
Your NHS salary is usually taxed through PAYE, but HMRC generally still expects a Self Assessment return where you have additional untaxed income or gains above the relevant reporting levels. The private element is reported on the return, and the tax already paid through PAYE is credited against your overall liability. The complication tends to appear in payments on account and in the accuracy of your tax code, which is why doctors with a growing private practice often benefit from a review partway through the year rather than only at filing time.
Locum doctors are usually treated as self-employed for tax purposes, which means reporting income on Self Assessment, keeping records of every session and payment, and retaining evidence for the costs of doing the work. Travel between sites, agency fees, indemnity cover, professional subscriptions and equipment can all be relevant, subject to the usual rules. If you work through a limited company or an agency, off-payroll working rules may also apply to some engagements, so it is worth checking how each contract is structured rather than assuming the position is the same across the board.
The general principle is that a cost must be incurred wholly and exclusively for the purposes of the work. In practice this often includes medical indemnity and defence union subscriptions, GMC and royal college or professional body fees, mandatory training and continuing professional development, specialist equipment and, where relevant, business mileage or travel between work locations. Costs with a personal element, such as home office use or a phone, usually need apportioning. Because the treatment depends on your employment status and the nature of the cost, it is worth reviewing your outgoings properly rather than assuming a standard list applies.
There is no single right answer, and the choice depends on your income level, how many people you work with, how much private or commercial activity you undertake, and what you plan to do over the next few years. Sole trader status is straightforward and suits doctors with modest additional income; a partnership is common in general practice and shared private practice; a limited company can suit certain private and consultancy arrangements, though it brings filing, payroll and possibly off-payroll working considerations. Many medical services provided by registered health professionals are exempt from VAT, and NHS income generally sits outside the scope, so registration is often not required. However, VAT depends on the nature and level of what you supply, and non-medical or commercial activities — such as certain reports, medico-legal work, product sales or training — can change the picture. If your turnover from taxable activities approaches the registration threshold, it is worth reviewing the position in advance rather than after the fact.

Switching tends to be more straightforward than doctors expect. We ask for details of your current position, your existing accountant is contacted for professional clearance and the relevant records, and we review what is in place before taking over the filing obligations. If your accounts or returns are already overdue, that is worth raising early so the position can be handled properly. Once the handover is complete, you have a single point of contact for your accounting and tax matters going forward.
