Not every doctor does. If you are employed on a straightforward NHS contract, pay the correct amount of tax through PAYE and have no other income, your tax may be handled for you. An accountant tends to become worthwhile once your circumstances add layers: private fees, locum shifts, consultancy income, a partnership share, a limited company, pension complications or a mix of these. Even where a return is not strictly required, doctors often value having their position reviewed so that allowable expenses are not missed and future decisions are better informed.
Locum work is typically treated as self-employment, which means you generally need to report your income and expenses on a Self Assessment return and may need to register with HMRC beforehand. Because locum work often involves multiple engagements and travel between sites, the records you keep matter: dates worked, locations, fees received and expenses incurred. Where locum work sits alongside an NHS salary, both sets of income must be reported correctly on the same return. The precise treatment can depend on the terms of your engagements, so it is worth reviewing before your first return.
Expenses must be incurred wholly and exclusively for the purposes of your work, and the position differs between employment and self-employment. Commonly considered items include medical indemnity insurance, GMC and specialist registration fees, professional body subscriptions, professional journals, training and professional development costs, equipment and uniforms. Travel and mileage may be claimable in certain circumstances, but ordinary commuting is generally not allowable. Because eligibility depends on your specific role and how the expense relates to it, we review claims individually rather than applying a blanket list.
In many cases yes, because private fees are not taxed through PAYE and need to be reported. The return must bring together your NHS employment income, any private or locum income and any other earnings, and the tax already deducted through PAYE is then offset against the total liability. Where income is significant, payments on account may also apply. The exact position depends on your total income and how it is earned, so a review of your circumstances is the sensible starting point.
There is no single answer. Sole trader status is simpler administratively, while a limited company introduces company accounts, Corporation Tax, payroll and often VAT obligations, and changes how profits can be extracted. The right structure depends on your level of profit, whether you have private or consultancy income, your pension arrangements, your attitude to administration and your longer-term plans. Tax treatment also changes with legislation, so a structure that suited you a few years ago may no longer be the most appropriate. We look at the numbers and the practicalities before recommending anything.
Pension contributions can have a meaningful effect on your taxable income depending on how they are made and whether you are employed, self-employed or both. NHS pension arrangements and any private contributions sit alongside your other income, and the interaction can raise questions about relief and about how contributions are treated across multiple income sources. Because the rules depend on your individual circumstances and can change, pension matters are best reviewed as part of your wider tax planning rather than in isolation.
Yes. Much of the information we need — payslips, invoices, bank statements, practice records and expense receipts — can be shared securely online, and meetings can be held by video or telephone at times that fit around clinics and rotas. Many doctors find remote working more practical than travelling to an office, and it also means your records stay in one organised digital place. If you prefer to meet in person, that can be arranged as part of the initial discussion.
Changing accountants is usually more straightforward than doctors expect. We would ask for your permission to contact your current accountant and request the handover of your records, accounts, tax returns and any outstanding correspondence. Once we have that information, we review your position, identify anything that needs attention and confirm what filings or payments are due. Where a return is already in progress, we coordinate with your existing accountant so that deadlines are not disrupted during the transition.
