Not every doctor does. If your only income is salaried NHS or hospital pay taxed through PAYE and your affairs are straightforward, you may not need ongoing support. However, an accountant is often worth considering once you have income that is not taxed at source, such as private fees, locum payments, consultancy income, medico-legal work, rental income or investment income. The same applies if you are a GP partner, run a practice or company, employ staff, or need to claim professional expenses accurately. It is less about whether you are a doctor and more about how many moving parts your finances contain.
It depends on your circumstances. Many doctors with only PAYE income do not need to file. However, a return is usually required where you have untaxed income such as private practice fees, locum earnings paid without tax deducted, consultancy or medico-legal income, partnership profits, significant dividend or rental income, or where HMRC has issued a notice to file. Higher earners with certain tax charges can also fall within Self Assessment. If you are unsure whether you meet the criteria, it is worth checking before the filing deadline rather than after it.
Locum income is usually treated as self-employment income, which means it is reported through Self Assessment and taxed after allowable expenses are deducted. That said, the position can differ depending on whether you work through an agency, are paid directly by a trust or practice, or provide your services through your own limited company. Where a company is involved, off-payroll working rules may affect how the engagement is treated. Keeping accurate records of shifts, travel and expenses throughout the year makes the reporting process significantly easier.
Commonly claimed items include medical indemnity and defence union subscriptions, GMC and specialist register fees, royal college and professional body memberships, relevant training and continuing professional development, specialist equipment and textbooks, and travel between work locations where it is not ordinary commuting. Whether a particular cost is allowable depends on your employment status and how the expense relates to your work, and the rules differ between employment and self-employment. Because of that, it is worth reviewing your expenses individually rather than assuming a standard list applies to everyone.
There is no single answer that suits every doctor. A limited company can be appropriate for some private practitioners, locums and consultants, but incorporation brings additional administrative obligations such as annual accounts, Corporation Tax returns, payroll and potentially VAT, and it does not suit every working pattern. Off-payroll rules, contract terms and the level and consistency of your income all need to be considered. The right approach is to look at your circumstances as a whole and weigh the practical benefits against the extra compliance involved.
Many medical services are exempt from VAT, which is why a significant number of doctors never need to register. However, not everything a doctor does falls within that exemption. Some non-medical services, certain cosmetic or aesthetic treatments, medico-legal reporting, the sale of goods or the provision of administrative services can be treated differently, and a mixture of exempt and standard-rated income can create registration and reporting questions. If your work includes any of these elements, it is worth reviewing your position carefully.
Yes. Bookkeeping, payroll, VAT returns and annual accounts can all sit alongside your Self Assessment, which is often more efficient than handling each task separately. Keeping records up to date through the year means your year-end figures are ready when needed and your tax position is clearer in advance. For doctors employing practice staff, locums or administrative support, payroll can also cover PAYE, payslips and the associated reporting obligations.
