Accounting and Tax Challenges Doctors Commonly Face
Medical careers rarely follow a tidy tax pattern. Income arrives from several directions, expenses are often professional rather than obviously business-related, and the rules around relief are not always intuitive. These are the areas doctors most often need help navigating.
- Multiple income streams in one tax year. A doctor might receive NHS salary through PAYE, locum income from several trusts, private consultation fees and occasional teaching payments — all in the same twelve months. Each source may be taxed differently, and without a consolidated view it is easy to underpay or overpay tax, or to miss a filing obligation altogether.
- Self Assessment alongside PAYE. Paying tax through PAYE does not automatically mean no tax return is needed. Untaxed income, higher-rate relief claims, pension annual allowance considerations and child benefit charges can all trigger a return. Missing the registration or filing deadline can lead to penalties that are entirely avoidable.
- Allowable professional expenses. Medical indemnity cover, GMC and royal college subscriptions, specialist equipment, professional journals, training and CPD costs may all be claimable in the right circumstances — but only where they relate to the work and meet the rules. Claiming too little leaves money on the table; claiming too much invites scrutiny.
- Travel and mileage records. Travel between sites, home visits, locum assignments and training days can add up quickly, but the relief available depends on whether the travel is ordinary commuting or genuinely business-related. Poor record keeping here is one of the most common reasons for a claim being reduced.
- Locum and consultancy income. Self-employed locum work brings its own obligations: registering with HMRC, keeping records, understanding expenses, and in some cases considering IR35 or off-payroll rules where engagements are structured through an intermediary.

The NHS Pension Scheme and private pension arrangements interact with tax in ways that can be complex, particularly around annual allowance charges and the timing of contributions. - Choosing and reviewing a business structure. Sole trader, partnership or limited company each carry different tax, administrative and liability consequences. What suited you when you were predominantly salaried may no longer fit once private or locum income grows.

At that point, the risks are practical: missing a filing obligation, claiming the wrong expenses, or paying more tax than necessary. PAYE collects tax on your employment income, but it cannot account for everything.
