If you are employed on a straightforward NHS contract with no other income, your PAYE position may be relatively simple, although it is still worth checking that your tax code is correct, particularly if you have pension contributions or professional expenses. If you take on locum shifts, private patients, consultancy work, medico-legal reports or a practice partnership, you will generally need to report that income through Self Assessment, and an accountant can help ensure it is calculated and recorded accurately.
Locum work is usually treated as self-employment, which means you report your income and expenses on a Self Assessment tax return and pay Income Tax and Class 2 and Class 4 National Insurance as applicable. Expenses that are wholly and exclusively related to the locum work, such as travel between sites, indemnity cover and professional subscriptions, may be allowable, but the position depends on the specifics of your arrangements, including whether an agency treats you as employed or self-employed.
