Yes. Exempt healthcare services do not carry VAT, but that does not necessarily mean you should ignore VAT altogether. Where a practice also supplies standard-rated items or non-clinical services, partial exemption rules may apply, and the way you calculate recoverable VAT needs to be consistent and defensible. Registration thresholds, scheme choice and the treatment of different activities can all affect the outcome. We review what you actually supply and advise on the most sensible approach rather than applying a default.
We set up and run payroll for salaried clinicians, administrative and reception staff, and where relevant associates or locums, making sure tax codes, National Insurance categories, pension contributions and payslips are handled correctly each period. The trickier part is often determining the correct status of individuals engaged on a self-employed basis, since the working arrangements matter more than the label used. We review those arrangements and flag where a different treatment may be needed, which helps protect both the practice and the individual concerned.
At a minimum, keep records of all income received and all expenses claimed, including invoices, receipts, bank statements, mileage logs and contracts or engagement letters. Digital records kept throughout the year are far easier to work from than a box of paperwork assembled in January. HMRC expects records to be retained for a set period after the relevant filing deadline, and where VAT or company accounts are involved the requirements differ slightly.
