The work covers both routine compliance and ongoing advice. In practice, that means preparing and filing Self Assessment returns, annual accounts and Corporation Tax computations, keeping bookkeeping up to date, running payroll and pensions for practice staff, handling VAT returns and partial exemption calculations where you have a mix of exempt and standard-rated supplies, and preparing management accounts so you can see how the practice is performing. Just as importantly, it means being available for questions — whether a new contract is worth signing, whether it is time to incorporate, or how much to set aside for an upcoming tax bill.
Not everyone, but far more healthcare professionals do than they might expect. Employed clinicians whose only income is taxed through PAYE may not need to file, but a return is usually required if you also earn private fees, do locum or agency work, receive income from medico-legal reports or teaching, have rental income, or your earnings exceed the thresholds HMRC applies. High earners may also need to file to account for the High Income Child Benefit Charge or pension annual allowance issues. The safest approach is to have your circumstances reviewed rather than assume you are outside the system.
All of it is taxable, but the way it arrives matters. NHS income may come through payroll with tax already deducted, while private fees typically arrive gross and need to be declared in full. The complexity comes from keeping the two separate in your records, working out which expenses relate to which activity, and making sure payments on account reflect your actual liability rather than last year's. Getting this wrong tends to surface later as an unexpected bill, which is why we reconcile income sources carefully before a return is submitted.
Broadly, costs that are incurred wholly and exclusively for your professional work. That commonly includes professional indemnity insurance, registration and revalidation fees, professional body subscriptions, specialist equipment and clinical clothing, medical references and textbooks, and a reasonable business proportion of costs such as phone, home broadband or a home office where you genuinely use it for work. What can and cannot be claimed depends on your role and how the expense was used, so it is worth reviewing each year rather than relying on a generic list.
It depends on your turnover and, crucially, on what you supply. Many healthcare services are exempt from VAT, which means they do not count towards the registration threshold in the same way standard-rated sales do. However, once you add standard-rated income — such as cosmetic treatments, non-medical reports, retail sales or commercial room hire — registration can become compulsory. If you are registered and supply a mixture of exempt and taxable services, partial exemption rules apply and some of the VAT you pay on costs may not be recoverable, so the calculations need to be done properly.

Employment status is part of the same conversation, because whether someone is genuinely self-employed or should be treated as an employee affects tax, National Insurance and, in some cases, the practice's exposure to penalties.
Accounting work is largely digital, so records can be shared securely and meetings held by phone or video at times that fit around clinics, surgery lists and home visits. Remote working suits many healthcare professionals who cannot easily step away during the working day, and it makes no difference to the level of support you receive. What matters is that your records are complete, your deadlines are known and your adviser understands how your income streams fit together.
